Rating The Intensity Of Pain

What is Tramadol

In addition to characterizing the chronicity and quality of pain, it is also important to assess the intensity of pain for proper pharmacologic selection. Pain intensity may be measured using one of three major pain rating scales including verbal, numerical and visual analogue scales. The McGill Pain Questionnaire developed by Ronald Melzack is the most widely used verbal pain scale since its introduction in 1975 . The questionnaire lists 20 groups of words that are used to describe and rate the intensity of pain. The 20 groups of words chosen are divided into four major groups to describe sensory qualities , affects , overall evaluation and other miscellaneous characteristics . The higher the score obtained out of 78 maximum points, the greater the pain. The Wong-Baker faces pain rating scale is another widely used system to rate pain intensity. It involves pictures of a smiling face indicating the absence of pain to severe facial grimacing and tearing for the worst pain . The Wong-Baker pain rating scale is especially useful for paediatric patients and those with poor verbal communications. Finally, a numerical rating scale consisting of a range of numbers, typically from 0 to 10, is probably one of the most widely used systems. A numerical rating scale is generally based on a subjective 10-point scoring system, where 0 denotes the absence of pain and 10 the worst pain imaginable.

Is Tramadol Bad For Your Liver Or Kidneys

Side effects like increased liver enzymes, hepatitis and liver failure have been reported in people using tramadol, but it is not known if tramadol caused these side effects directly. How often these side effects may occur are not known.

In addition, if you already have kidney or liver disease, your dosage of tramadol may need to be adjusted. Tramadol is eliminated from your body by both the kidneys and liver, so you need good kidney and liver function to use this medicine.

Tramadol: A Rare Cause Of Acute Interstitial Nephritis

Discussion: Tramadol is a centrally acting opiod analgesic used for the treatment of moderate to severe pain. It binds to the u-opiate receptor causing inhibition of central pain pathway. It is extensively metabolized in the liver and excreted by the kidney partly in the unchanged form and the rest as a metabolite. It causes a rise in BUN and/or creatinine in < 1% of the cases and is rarely implicated in the etiology of AIN. AIN is a renal lesion that is characterized by inflammatory interstitial infiltrate causing decreased creatinine clearance and usually confirmed by a renal biopsy. Although antibiotics and non-steroidal anti-inflammatory medications have been classically implicated in the pathogenesis of AIN, virtually any drug including Tramadol, as illustrated in this case, can cause AIN. If the kidney function does not improve after discontinuing the offending agent, corticosteroid therapy may be beneficial.

Is Tramadol Safe For Children

200mg Tramadol

Only a doctor can determine if tramadol is safe to use in a child.

  • Tramadol should NOT be used in children younger than 12 years of age.
  • Tramadol should NOT be used in children younger than 18 years of age following a type of surgery to remove the tonsils and/or adenoids.
  • Tramadol should be avoided in adolescents 12 to 18 years of age who have other risk factors that may increase their sensitivity to breathing problems while taking tramadol.

Life-threatening respiratory depression and death have occurred in children who received tramadol. Some of the reported cases followed surgery to remove the tonsils and/or adenoids. In at least one case, the child had evidence of quickly breaking down tramadol in their body due to an alteration in enzymes called CYP2D6.

ABSTRACT:Opioid analgesics are considered the mainstay of pain management. The primary goal of analgesia is optimizing the patients comfort. However, dosing these agents may be complicated in patients with renal and/or hepatic impairment. Most recommendations are based on case reports therefore, appropriate analgesic agent selection requires a thorough understanding of the drugs pharmacokinetics and side-effect profiles. This article will review opioid analgesics and their vital clinical pharmacokinetic considerations when treating patients with hepatic and/or renal insufficiency.

Pain is a condition affecting more Americans than diabetes, heart disease, and cancer combined, with an estimated incidence of 100 million people.1 Comorbid conditions such as hepatic and renal disease may complicate pain treatment with appropriate opioid analgesics.

Does Tramadol Cause Liver Or Kidney Damage

Tramadol is metabolized, or broken down, by your liver. This means theres a chance it can cause liver damage. However, liver damage is not common if you take tramadol at typical, prescribed doses. If they do occur, they are often mild and resolve on their own.

Because theres a slight risk of liver damage from tramadol, your doctor will do a baseline liver function test before you start taking this medication. If youre taking tramadol long-term, they will continue to monitor your liver function to make sure everything is okay.

Tramadol is not directly toxic to your kidneys, but it can be dangerous for people with chronic kidney disease.

Tell your doctor if you have liver or kidney disease. They may choose a different medication for you, or they may give you a lower dose or space out your doses.

Liver and kidney damage are both more likely if you take high doses of tramadol. In addition, if you overdose on tramadol, that can cause acute liver or kidney failure. Thats one more reason to only take tramadol as prescribed.

The management of pain in patients with CKD and ESKD similarly follow the WHO 3-step ladder approach, albeit with special considerations due to altered drug pharmacokinetics and various physiological aspects associated with reduced kidney function.

Increased drug levels and associated adverse effects may occur due to reduced renal clearance and accumulation of a toxic parent compound and/or its metabolite or increased free drug levels due to reduced protein binding associated with hypoproteinemia/hypoalbuminemia and/or acidemia . Drug removal by various modes of dialysis must also be considered.

Tramadol Can Be Habit

Tramadol is structurally related to the opioids like codeine and morphine and can increase your risk for misuse, abuse, addiction, overdose and death, even if you take the medicine exactly as prescribed by your doctor. Never share tramadol with anyone else, as they could die from taking it.

Talk to your healthcare provider about naloxone , a medicine you can keep on hand or at your house for emergency treatment of an overdose. It is important to have Narcan on hand if you take any opioid medicine. Ask your pharmacist or doctor about getting naloxone to help reverse an overdose emergency.

If you use tramadol on a regular basis, do not abruptly stop taking it on your own without talking to your doctor first. Serious side effects like nausea, diarrhea, anxiety, sweating, trouble sleeping, shivering, pain, tremors, or rarely, hallucinations may occur.

Keep your tramadol in a safe and out-of-sight place at home to prevent theft, accidental overdose or death. Keep out of the reach of children and pets. Dispose of unused tramadol in a Drug Take-Back Program. If one is not available, your pharmacist can tell you the best way to safely dispose of tramadol.

Chronic Pain In Ckd Patients

Chronic pain is defined by Treede et al. as pain that persists past normal healing time and hence lacks the acute warning function of physiological nociception . It may be due to prolonged tissue injury with persistent activation of nociceptors or other undefined mechanisms .

Recommendations of the WHO for cancer pain and its three-step approach can be tailored to manage pharmacologic treatment of chronic pain in CKD patients . In patients with mild pain, acetaminophen may suffice, and if NSAIDs are required, care should be taken to avoid other medications which worsen the hemodynamics . NSAIDs should be avoided in patients with impaired cardiac output or with dehydration.

For moderate pain, weak opioid tramadol may be added, but codeine and dihydrocodeine should not be prescribed. Tramadol dose is adjusted according to CKD stage, given in regular doses for stages 1 and 2, but is reduced by 50% in advanced stages of CKD.

Severe pain requires the use of strong opioids, preferably those without active metabolites such as methadone, fentanyl, and oxycodone. Although, oxycodone is mainly metabolized in the liver, around 20% is eliminated by kidneys. For that reason, it may be wise to reduce the dose according to CKD stage in mild renal failure, up to 50% of normal dose should be given, and in advanced stages, 25% of normal dose and increase in dosing interval should be done. In patients with consistent pain, fentanyl patch can be a good option .

What Are the Common Losartan Side Effects

Brown S, Lascarides P, Stickevers S. Treating Pain in Patients With Chronic Kidney Disease: A Review of the Literature. Pract Pain Manag. 2014 14.

Patients with chronic kidney disease often suffer from chronic pain. It may be difficult to select appropriate analgesic therapy in this population because many patients require complex medication management for the comorbidities that accompany renal disease. A reduced glomerular filtration rate alters the normal pharmacokinetics of analgesic medications and increases the potential for toxicity, undesirable side effects, and drug interactions. Appropriate analgesic selection, dose titration, and monitoring are critical for the successful management of this population.
