  • Magnetic resonance imaging : Magnets, radio waves and a computer create images of inside the body. They also can be used to tell the difference between cysts filled with fluid and solid masses. Because they don’t require iodinated contrast, MRIs are used for patients with iodine allergy.

Diagnosing Simple Kidney Cysts

Simple kidney cysts are often first found with an imaging test that was done for another reason. Your healthcare provider will ask about your health history and symptoms. He or she will also give you a physical exam.

It is important to tell simple kidney cysts from complex cysts. Complex cysts are a different kind of cyst that may be cancer. A complex cyst needs to be removed. For this reason, you may need tests such as:

  • Kidney ultrasound

  • Kidney CT scan, if a detailed picture of the cyst is needed

  • Kidney MRI, if more information is needed about the cyst

A radiologist will look at these pictures to see if your kidney cyst is simple or complex. A cyst may be rated with the Bosniak CT system. This has 5 categories based on how the cyst looks. If your cyst is a category 1, you likely wont need any more tests. A kidney cyst with a higher rating may need more tests or treatment. A category 5 cyst is most often linked with cancer.

Your healthcare provider will also check for other conditions that may be causing the cysts. You may need to have genetic testing. It can find other problems, such as polycystic kidney disease.

What Is A Kidney Cyst

There are different kinds of kidney cysts. A simple kidney cyst is a round pouch of smooth, thin-walled tissue or a closed pocket that is usually filled with fluid. One or more may form within the kidneys. Simple cysts are the most common type of kidney cyst, and they most often donât cause harm.

Simple kidney cysts arenât related to polycystic kidney disease . PKD runs in families and causes large numbers of cysts to grow in your kidneys. This makes your kidneys get larger and damages their tissue.

Over time with PKD, your kidneys donât work as well, and the disease can lead to kidney failure. Sometimes PKD can cause cysts to form in your liver or in other parts of your body.

Acquired cystic kidney disease is another condition that causes cysts to grow in your kidneys. It doesnât run in families like PKD, but happens in adults and children with chronic kidney failure and end-stage renal disease. ACKD doesnât cause your kidneys to grow larger or lead to cysts in other parts of the body. It usually doesnât cause symptoms or need treatment.

How Is Pkd Treated

At present, there is no cure for PKD. However, a lot of research is being done. Recent studies suggest that drinking plain water throughout the day and avoiding caffeine in beverages can slow the growth of cysts. Research is also helping us understand the genetic basis of PKD.

Studies also suggest that some treatments may slow the rate of kidney disease in PKD, but further research is needed before these treatments can be used in patients. In the meantime, many supportive treatments can be done to control symptoms, help slow the growth of cysts, and help prevent or slow down the loss of kidney function in people with PKD. These include:

  • careful control of blood pressure
  • prompt treatment with antibiotics of a bladder or kidney infection
  • lots of fluid when blood in the urine is first noted
  • medication to control pain
  • a healthy lifestyle with regard to smoking cessation, exercise, weight control and reduced salt intake
  • drinking lots of plain water throughout the day
  • avoiding caffeine in all beverages

In April 2018, the FDA approved a new drug called tolvaptan for the treatment of autosomal dominant polycystic kidney disease . The drug can be used to help slow kidney function decline in adults at risk for this type of PKD. Should People With Pkd Take A Special Diet

At present, no specific diet is known to prevent cysts from developing in patients with PKD. Reducing salt intake helps control blood pressure in PKD patients who have high blood pressure. A diet low in fat and moderate in calories is recommended to maintain a healthy weight. Speak to your doctor or a dietitian about other changes to your diet, such as avoiding caffeine.

Eat less sodium!

Low-sodium diet is important for people with kidney problem, even though if you only have simple kidney cysts. High amounts of sodium in the circulation will make the kidneys work harder than usual. Even for healthy people with healthy kidneys, diet high in sodium is still dangerous and not recommended.

Too much sodium that you eat is also often to blame for high blood pressure, a common cause and a common consequence of kidney disease. If you have fluid retention and swelling in legs, sodium can also worsen these symptoms.

However, sodium is not always bad. In fact, the body also needs it to support some body functions. Just make sure to eat it in moderation, not more than 2.4 gram of sodium per day!

The daily recommended maximum of sodium in children varies, depending on the ages. But in general, children should eat less sodium than adults.

However, daily clinical practice shows that with careful observation, renal cysts are rarely completely devoid of internal echo. This is due to the artifacts created by the propagation of the ultrasonic wave through the tissues.

What Is Kidney Cyst

Simple kidney cysts are abnormal, fluid-filled sacs that form in the kidneys 1). Simple kidney cysts are different from the cysts that develop when a person has polycystic kidney disease, which is a genetic disorder. Simple cysts are usually unilateral and solitary lesions with well-defined features. Simple kidney cysts do not enlarge the kidneys, replace their normal structure, or cause reduced kidney function like cysts do in people with polycystic kidney disease.

Simple cysts may be present at birth. Ultrasonography on approximately 30,000 fetuses in succession revealed an incidence of 0.09%, in most of whom the cysts resolved by birth 2). Only two of them persisted as simple benign cysts and in a third case heralded a unilateral multicystic dysplastic kidney. Between birth and 20 yr of age, the occurrence of new cysts is very rare but thereafter begins to increase in frequency, with an increased male-to-female ratio of approximately 2:1 in some studies 3). The reported overall prevalence of simple cysts is variable. Depending on the population and method of study, reported prevalences range from 5 to 41% but are likely in the range of 7 to 10%. Older autopsy studies reported their presence in 3 to 5% of cases 4).

Simple kidney cysts are more common as people age. An estimated 25 percent of people 40 years of age and 50 percent of people 50 years of age have simple kidney cysts 5).

Renal Mass: Cysts To Renal Cell Carcinoma

Most renal masses are simple cysts, frequently found incidentally on US, CT, and MRI. They rarely occur in individuals younger than 25 years of age, but are found in more than 50% of patients older than 50 years of age. Typically, renal cysts are asymptomatic and cortical in location they may be single or multiple. Their cause is unknown, although tubular obstruction has been postulated to be a necessary element.

Renal masses produce variable findings on imaging studies, depending on their location. For years IVU was the imaging modality of choice for detection of renal masses however, the findings on IVU are frequently nonspecific, and further imaging is necessary to characterize most abnormalities found . Studies have shown that IVU has low sensitivity for detection of renal masses, especially those smaller than 3cm in diameter. 237 With CT as the gold standard, IVU detected 10% of masses smaller than 1cm in diameter, 21% of masses 1 to 2cm in diameter, 52% of masses 2 to 3cm in diameter, and 85% of masses larger than 3cm in diameter. 237 US fared better than IVU but detected only 26% of masses smaller than 1cm, 60% of those 1 to 2cm, 82% of those 2 to 3cm, and 85% of those larger than 3cm. 237

The findings on IVU are nonspecific, and US, CT, and MRI are used to characterize the renal mass, differentiating solid from cystic.

Risk Factors And Associations

Several studies report advancing age , male gender , renal dysfunction , hypertension and smoking as associated with renal cysts on multivariate analysis. One study has reported the ipsilateral coexistence of renal stones with Stage I renal cysts on multivariate analysis, with an odds ratio of 2.15 . There are several limitations to these studies including the definition of hypertension failure to analyse Stage I renal cysts separately from more complex cysts or inherited cystic kidney disease and failure to exclude confounding factors. Whether the apparent associations between Stage I renal cysts and the aforementioned factors particularly renal dysfunction and hypertension are coincidental or genuine remains controversial and will be discussed below.

A possible pathogenic link between smoking and Stage I renal cysts has been proposed: smoking could be either directly toxic or induce renovascular disease causing relative renal ischaemia. However, there is conflicting evidence from studies from South East Asia either supporting or refuting smoking as an independent risk factor for the development of Stage I renal cysts.
