While the study was conducted on people with kidney disease, we could safely extrapolate the recommendations to those who want to avoid kidney disease and achieve optimal kidney function now, especially as we age.

In fact, additional research points to the actuality of physiological changes in the kidneys as we age. The research notes that a progressive reduction of the glomerular filtration rate and renal blood flow are observed in conjunction with aging. The reason for these phenomena is a decrease in the plasma flow in the glomerulus, a bundle of capillaries that partially form the renal corpuscle.2

In addition, the aging kidneys experience other structural changes, such as a loss of renal mass, and decreased responsiveness to stimuli that constrict or dilate blood vessels. The study concludes with a notable summation:

What Other Testing Might Be Required

What, Why, and How of a Kidney Function Test

Depending upon your pet’s clinical signs, and the results of the initial screening tests, further testing for kidney disease may include any of the following:

A urinary protein/creatinine ratio is recommended if the urine dipstick evaluation suggests that an excessive amount of protein is being lost in the urine. A urine sample is sent to a veterinary referral laboratory, and the ratio of protein to creatinine in that sample is calculated. An increased protein/creatinine ratio can indicate kidney damage.

Urine culture may be performed if the urinalysis findings are suggestive of a bacterial infection as the cause of the kidney disease.

Leptospirosis titers may be recommended if this organism is suspected of causing the kidney disease. See handout Antibody Titers for more information.

Occasionally ultrasound examination and/or kidney biopsy may be recommended to evaluate the kidneys.

At NYU Langone, specialists called nephrologists diagnose and manage kidney disease. In this chronic condition, the kidneys slowly lose their ability to filter waste from the blood. If this condition isnt managed, it can lead to kidney failure, in which one or both kidneys no longer function. Some people may experience acute kidney failure, in which the organs fail in less than two days.

The kidneys are small, bean-shaped organs, each about the size of a fist, located under the rib cage in the back. They have several important functions, including removing digested food, chemicals, medication, and other waste products from the blood. These are then excreted in urine.

The kidneys also balance the bodys levels of fluids and electrolytes, such as phosphorus, potassium, and sodium. These are essential for the proper functioning of the heart, muscles, and other structures. The kidneys also make hormones that help regulate blood pressure, stimulate the production of red blood cells, and strengthen bones.

When the kidneys are not able to function properly, the body retains toxins and excess fluid and can fail to make enough red blood cells, which carry oxygen to organs and tissue.

Some people with chronic kidney disease experience no symptoms. They may receive a diagnosis only after having blood or urine testing for another condition.

When Should I Get A Renal Panel Test

A renal panel test can be used in a range of circumstances, and the doctor may include specific measurements depending on your situation.

As a diagnostic test, a renal panel is most frequently used when you have symptoms that could be explained by a kidney problem. Examples of symptoms that can be tied to kidney impairment or disease include:

  • Urinary changes including changes to the frequency, quantity, or appearance of your urine
  • Unexplained swelling, especially in your arms, hands, legs, and/or face
  • Itching
  • Fatigue
  • Muscle cramps or pain

A test like the renal panel may also be used for diagnosis when you have general symptoms without a clear cause or are being evaluated in an urgent care or emergency setting.

As a screening test, the renal panel or other tests of kidney health are most often prescribed if you have certain risk factors for kidney disease. Some of these risk factors include high blood pressure, diabetes, and a family history of kidney disease.

For people with an elevated risk of kidney problems, screening with a renal panel may be part of normal health checkups. Screening of this type is not standard in people who do not have risk factors.

A renal panel can be used as a monitoring test if you have had kidney problems in the past or have already had an abnormal renal panel test. If you are receiving treatment for kidney injury or disease, a renal panel may be used to gauge your response to that therapy.

What To Expect After Blood Tests

Once the needle is withdrawn, you’ll be asked to apply gentle pressure with a piece of gauze or bandage to the place where the needle was inserted. This helps stop bleeding. It also helps prevent swelling and bruising.

Most of the time, you can remove the pressure after a minute or two. You may want to keep a bandage on for a few hours.

Usually, you don’t need to do anything else after a blood test. Results can take anywhere from a few minutes to a few weeks to come back. Your doctor should get the results. It’s important that you follow up with your doctor to discuss your test results.

Urine Tests For Kidney Disease

Damaged or inflamed kidneys leak substances such as blood or protein into the urine. The preferred test for detecting protein in the urine is a urine albumin-to-creatinine ratio test, which shows the amount of albumin in the urine.

A urine ACR test should be done at least once a year if the person has diabetes or high blood pressure, and every two years if the person has any of the other identified risk factors for developing chronic kidney disease.

A urine ACR test is performed by sending a sample of your urine to a laboratory for analysis.

How Is A Renal Panel Test Different From An Estimated Glomerular Filtration Rate Test

Kidney Failure Risk Factor: Urine Albumin

The renal panel is a broader test than an estimated glomerular filtration rate test, and in some cases, a renal panel involves eGFR.

The estimated glomerular filtration rate is a common method of evaluating kidney function. It can be calculated in several different ways and reflects how much blood is being filtered by the kidneys every minute.

The eGFR is usually determined with a mathematical formula based primarily on the amount of creatinine in the blood. Because creatinine is measured in most renal panels, the test report may also list eGFR. However, the renal panel also measures electrolytes, minerals, and other substances that can reflect the status of the kidneys.

Estimated Glomerular Filtration Rate

Estimated glomerular filtration rate provides a guide to kidney function. Although the level of creatinine in the blood is a useful guide to kidney function, the eGFR is a more accurate measure. Blood creatinine can be used to estimate the eGFR using age, sex and race. This is often calculated by computer and reported with the creatinine blood test. The normal value for eGFR is 90-120 ml/min. An eGFR below 60 ml/min suggests that some kidney damage has occurred. The value becomes lower with increasing severity of kidney damage.

In order to understand kidney function tests, also known as a renal panel, it is important to understand what the kidneys do. The kidneys filter the blood and remove unwanted waste from the bloodstream. They also remove excess water from the body, helping to regulate blood pressure and the fluid balance of the body.

When the kidneys are functioning well, there is very little unwanted waste in the body. When these waste levels in the blood begin to climb, they can indicate that the kidneys are no longer functioning as well as they should.

Biopsy For Kidney Disease

A biopsy means that a small piece of tissue is taken for testing in a laboratory. Biopsies used in the investigation of kidney disease may include:

Can I Take The Test At Home

How can I be tested for kidney disease?

Very few options exist for a complete renal panel test that can be taken with an at-home test kit. Some at-home kidney tests involve taking a fingerstick blood sample at home and sending it to a lab for analysis, but these tests do not include all of the same measurements that are included in a typical renal panel test.

Blood Tests For Kidney Disease

The best measure of kidney function is the glomerular filtration rate , which can be estimated from a blood test that checks the blood for creatinine .

A normal GFR result is higher than 90 mL/min/1.73 m2. If the result is persistently less than 60 mL/min/1.73 m2 for at least three months, this confirms that the person has chronic kidney disease.

Blood tests can reveal other abnormalities of kidney function, such as:

  • high levels of acids
  • anaemia
  • high levels of potassium
  • low levels of salt
  • changes to the levels of calcium and phosphate.

Treatment For Stages 1 2 3 And 4

If your child is diagnosed in stages 1, 2, or 3 of CKD, they probably won’t have many symptoms, but their kidneys won’t be functioning the way that they should, which can lead to complications.

If your child’s kidneys aren’t properly regulating the acid levels in the blood, this may result in a condition called acidosis, which doesn’t have any symptoms. We can treat acidosis with bicarbonate, an oral medicine that balances the acid levels.

The kidneys regulate the level of calcium and phosphorous in your child’s body. If they begin to lose the ability to do this, we can supplement those minerals with activated vitamin D, medicines that prevent the absorption of calcium and phosphorous, and regulating diet.

Your child’s blood pressure may start to go up, and if we treat this with blood pressure medication early on, we can minimize the risk of cardiovascular disease as the condition progresses.

If your child’s kidneys aren’t making erythropoietin , a hormone that regulates how the body makes red blood cells, she may develop anemia and experience weakness, fatigue and/or have trouble concentrating. To relieve these symptoms, we may recommend EPObe given as a shot at home, usually weekly or every other week. Many symptoms associated with chronic kidney disease are related to anemia, so this treatment has a very good chance of making your child feel better.

A lipoprotein panel is a blood test that can help show whether you’re at risk for coronary heart disease . This test looks at substances in your blood that carry cholesterol.

A lipoprotein panel gives information about your:

  • Total cholesterol.
  • LDL cholesterol. This is the main source of cholesterol buildup and blockages in the arteries.
  • HDL cholesterol. This type of cholesterol helps decrease blockages in the arteries.
  • Triglycerides. Triglycerides are a type of fat in your blood.

A lipoprotein panel measures the levels of LDL and HDL cholesterol and triglycerides in your blood. Abnormal cholesterol and triglyceride levels may be signs of increased risk for CHD.

Most people will need to fast for 9 to 12 hours before a lipoprotein panel.

How Do I Prepare For A Creatinine Blood Test

Treatment For Stage 5 Ckd

Stage 5 is defined as end-stage renal disease, at which point your child needs to go on dialysis or have a kidney transplant. Both are effective treatments, and our goal is to transplant virtually all of our patients with ESRD. Around 75 percent of children with ESRD go on dialysis before receiving a transplant.

One of the roles of your child’s kidneys is to act as a filter for their blood, making sure that it has the right balance of water and minerals. If your child’s kidneys are unable to do this, dialysis is a procedure that can do it for them. Dialysis may be given every night at home, or at a hospital or dialysis center three or four times a week.

In some children, including those with the severe form of FSGS or familial hemolytic uremic syndrome , the disease causes the kidney to fail almost as soon as it is transplanted. In other cases, it may not be a good time for the family to have the child undergo a transplant. The good news is that a kidney transplant is very rarely an emergency, and never absolutely necessary, because dialysis is such an effective treatment.
