In 2015, AstraZeneca said it would pay the U.S. government $7.9 million to resolve kickback allegations involving Nexium. The government claimed AstraZeneca worked with another company to boost sales.

Hidden financial agreements between drug manufacturers and pharmacy benefit managers can improperly influence which drugs are available to patients and the price paid for drugs, Joyce R. Branda, the Acting Assistant Attorney General, said at the time.

The federal government alleged AstraZeneca agreed to pay pharmacy-benefit manager Medco Health Solutions in exchange for Medco maintaining Nexiums sole and exclusive status on certain Medco lists that give details of medicines that may be prescribed. The government further alleged that AstraZeneca provided some or all of the payment to Medco through price concessions on drugs including Prilosec, Toprol XL and Plendil.

More than half of the people who take PPIs probably do not need them. Simple heartburn can be treated with antacids or other drugs, plus diet and lifestyle changes.

A team of medical researchers recently examined the so-called “mechanism of injury” for the long-term use of Nexium — and perhaps other heartburn or reflux medications in the proton pump inhibitor class of drugs — apparently causing an increased risk of kidney problems as well as heart attacks and dementia.

As background, there is a growing body of medical evidence which indicates that some popular heartburn drugs and acid reflux medicines like Prevacid, Prilosec, and Nexium can cause some serious kidney-related side effects. In more detail, the use of these proton pump inhibitors for even just a couple of weeks can result in an increased risk of developing:

How Do Ppis Treat Heartburn

Proton Pump Inhibitors work by shutting down the pumps that secrete stomach acid. Stomach acid is crucial in the digestion of food. However, too much of it is harmful to other organs in the digestive system. So when the stomach walls produce too much acid, PPIs block the enzyme that triggers its production.

When the PPIs shut down the acid production, the signs of gastroesophageal reflux disease, acid reflux, heartburn, or ulcers are reduced.

The effectiveness of PPIs is fast, and some people may tend to stop the dosage. The discontinuation of the medication may lead to a resurgence of acid production in more significant quantities.

PPIs do not treat the cause of the excessive production of stomach acid. They only block its production. Therefore, the condition may keep coming back.

Outcome Variable: Ckd Stage Evolution

In this study only patients with confirmed diagnosis of CKD were included, patients with acute kidney injury and other renal diseases were not evaluated.

The evaluation of CKD stage and the identification of omeprazole users and non-users occurred at the time of diagnosis in the ambulatory clinic, followed by subsequent consultations. For a large number of patients, the CKD stage was available and recorded by the prescriber in the medical record at the time of the consultation. When the CKD stage was not available, the eGFR was calculated and classified according to the KDIGO and MS guidelines using the CKD-EPI equation without correction for race . Evolution of CKD was considered when the stage changed to a more advanced stage of renal impairment between one consultation to another, considering an increasing scale of severity starting from stage 1. In addition to this analysis, for each time, a 25% or more reduction in eGFR compared to the eGFR of the previous time was evaluated. Plus, a rapid progression analysis was performed as recommended by KDIGO .

    The U.S. Food and Drug Administration has cautioned against using PPIs to treat simple digestive problems like heartburn, but that is a major reason people take this drug. The drug, however, was not designed to provide quick relief it is meant for longer term use for chronic problems. With that long term usage, chances for developing adverse side effects increases.     According to the American Society of Nephrology, over 20 million Americans suffer from chronic kidney disease today. Researches also have found that a quarter of kidney patients had used a PPI drug and that they then had twice the risk of dying prematurely. Another study of 10,000 adults found that PPI users were 50 percent more likely to develop chronic kidney disease than persons who did not take PPI drugs. This finding was not evident for persons who took a different class of drugs to suppress stomach acid, such as Tagamet or Zantac.

    Study Design And Population

    This is a retrospective cohort study developed at the nephrology ambulatory sector of the Municipal Polyclinic of Divinópolis-MG, Brazil, from March 2017 to March 2019, following the STROBE checklist for observational research. Eligibility criteria were defined as all adult and elderly patients of both genders with established CKD diagnosis who were already undergoing clinical follow-up or who were diagnosed after insertion in the nephrology ambulatory clinic in the years 2016 and 2017. During this period, 241 patients were attended. Of these, 42 patients were excluded due to lack of confirmation of CKD diagnosis , absent creatinine values and history of neoplasms . Patients without baseline creatinine values were excluded from the study because it would not be possible to determine the estimated glomerular filtration rate and thus the baseline CKD stage. Similarly, those patients who had not recorded serum creatinine values at subsequent times were also excluded. Patients who underwent previous kidney transplantation and/or patients on renal replacement therapy, and patients who had missing data related to some of the variables used during the two-year follow-up were not included in this study. Thus, a total of 199 patients with CKD eligible for the study were included.

    Nexium & Prilosec Lawsuit Settlement Amounts

    As of this time, there have been no large mass tort settlements involving Nexium or Prilosec and the potential link to kidney injuries. Generally, however, large groups of settlements do not occur until such time as a few lawsuits are tried before a jury, and the manufacturer is able to more thoroughly understand its financial risk.
