Sodium Bicarbonate May Mitigate Calcium Phosphate Stone Risk In Ckd

Sodium bicarbonate, which is commonly used to treat metabolic acidosis in patients with chronic kidney disease , is hypothesized to increase the risk for calcium phosphate kidney stones by increasing urinary pH. New study findings presented at the virtual National Kidney Foundation 2021 Spring Clinical Meetings suggest that although sodium bicarbonate does increase urinary pH, it significantly raises urinary citrate levels, which may mitigate the risk for calcium phosphate stones.

A team led by Kalani Raphael, MD, of Oregon Health & Science University in Portland, and colleagues studied 168 patients with CKD who participated in the randomized BASE Pilot Trial. Of these, 76 received higher-dose and 47 received lower-dose oral sodium bicarbonate, and 45 received placebo.

Compared with placebo, the higher-dose and lower-dose sodium bicarbonate groups experienced a significant mean 0.97 and 0.77 increase in urinary pH from baseline, Dr Raphaels group reported in a poster presentation. Urinary citrate/creatinine concentration increased significantly by a mean 145.7% and 108.3% in the higher-dose and lower-dose sodium bicarbonate groups, respectively. Sodium bicarbonate had no effect on urinary calcium or phosphate.

I dont think these findings should totally alleviate concerns about promoting calcium phosphate stones, Dr Raphael told Renal & Urology News, but we understand some of the chemical changes that happen with sodium bicarbonate a little bit better.

I am not at all sure why the pricing of potassium citrate has become a topic I often hear about from patients, doctors, and just about everybody in the kidney stone world. Certainly the price must have increased, but I cannot find data on the web to prove the point. I also believe Medicare and perhaps other insurers have altered the status of this drug in their payment schedules. Perhaps some of you know more about the problem than I do and are willing to share what you know by way of a comment.

I did find on inspection of the Medicare lists of drug prices by insurance plan that some plans appear to include potassium citrate pills in their formularies at a preferred level and charge as little as $10 for what appears to be 90 pills. Others do not do this and publish higher prices, often as percentages of the retail cash price. Once again, I hope those of you with experiences in purchasing the drug will share what you know.
