What Kind Of Scientific Study Was This
This randomised controlled trial investigated the effects of bicarbonate supplementation for people with chronic kidney disease and metabolic acidosis.
Metabolic acidosis is a condition where there is an acid-alkali imbalance in the blood, that results in high blood acidity and low plasma bicarbonate levels. Several conditions can lead to metabolic acidosis, including heart failure, drugs or toxins, kidney failure or diabetic ketoacidosis . It is a common complication in people with advanced chronic kidney disease, and it can interfere with protein metabolism and may lead to stunted growth and loss of bone and muscle.
The study looked at 134 patients with chronic kidney disease and low blood bicarbonate levels . The patients were randomly allocated to either sodium bicarbonate supplements, 600mg taken orally three times a day , or to usual care for two years.
The researchers defined rapid progression as a reduction of creatinine clearance of more than three ml/min per 1.73m2 per year.
How Does It Work
The endocrine system produces hormones and enzymes to help digest food into safer chemicals that the body can process. The pancreas is usually the organ that produces the majority of sodium bicarbonate to protect the kidneys during digestion. Sometimes, the kidneys will produce additional amounts, depending on the type of food you eat.
If your diet includes added sugar, fried foods, fatty foods and other unhealthy choices, the endocrine system can become stressed. Both the pancreas and the kidneys will suffer a significant reduction in their ability to produce sodium bicarbonate effectively. Without sufficient production of bicarbonate, the acids produced during digestion cannot be effectively neutralized, causing kidney damage.
In dialysis or other treatments for kidney damage, sodium bicarbonate is one of the most common agents used. A process known as acid buffering aids in the removal of excess fluids and waste material from the bloodstream. Dialysis helps to add sodium bicarbonate to the body, because the kidneys are no longer able to produce it in order to process food and waste materials. When the body is able to produce normal amounts of bicarbonate on its own, the life of the kidneys is extended, and the endocrine system can remain healthy.
Who Needs Sodium Bicarbonate
If sodium bicarbonate is considered important for a persons treatment, then their physician will prescribe a dose of sodium bicarbonate in a tablet form. It can help be a buffer to decrease acid level in the blood. Be aware this medication is high in sodium and should not be used without physician approval. Problems with metabolic acidosis are more likely present in late stages of chronic kidney disease and in dialysis patients.

Sodium Bicarbonate May Mitigate Calcium Phosphate Stone Risk In Ckd
Sodium bicarbonate, which is commonly used to treat metabolic acidosis in patients with chronic kidney disease , is hypothesized to increase the risk for calcium phosphate kidney stones by increasing urinary pH. New study findings presented at the virtual National Kidney Foundation 2021 Spring Clinical Meetings suggest that although sodium bicarbonate does increase urinary pH, it significantly raises urinary citrate levels, which may mitigate the risk for calcium phosphate stones.
A team led by Kalani Raphael, MD, of Oregon Health & Science University in Portland, and colleagues studied 168 patients with CKD who participated in the randomized BASE Pilot Trial. Of these, 76 received higher-dose and 47 received lower-dose oral sodium bicarbonate, and 45 received placebo.
Compared with placebo, the higher-dose and lower-dose sodium bicarbonate groups experienced a significant mean 0.97 and 0.77 increase in urinary pH from baseline, Dr Raphaels group reported in a poster presentation. Urinary citrate/creatinine concentration increased significantly by a mean 145.7% and 108.3% in the higher-dose and lower-dose sodium bicarbonate groups, respectively. Sodium bicarbonate had no effect on urinary calcium or phosphate.
I dont think these findings should totally alleviate concerns about promoting calcium phosphate stones, Dr Raphael told Renal & Urology News, but we understand some of the chemical changes that happen with sodium bicarbonate a little bit better.

Pros And Cons Of Carbonated Water
Generally speaking, Harvard Health Publishing says that soda water or seltzer is a much better nutritional choice than sugary sodas. High soda intake is linked to chronic health problems like Type 2 diabetes and heart disease, and diet soda is associated with health risks, as well.
Reviewing the pros and cons of carbonated water versus flat water can help you decide which beverage to go for. Like flat water, plain carbonated water contains zero calories. However, flavored versions may be higher in calories.
The carbonation of soda water is both a pro and a con. It can make you feel more full, which is helpful for satiety. However, the fullness may prevent you from drinking more, which is why flat water is a better option if you are dehydrated.
One potential link between carbonated water and kidney stones is the sodium content of drinks like club soda. The USDA estimates that a 12 fluid ounce can or bottle of club soda contains almost 75 milligrams of sodium, as well as 17 milligrams of calcium. In comparison, a 12 fluid ounce bottle of flat water provides just over 7 milligrams of sodium, 36 milligrams of calcium and 7 milligrams of magnesium.

Prevention Of Kidney Stones
In Australia, the chance of forming a kidney stone is becoming more common. It has been estimated there is a 1 in 12 chance you may develop a stone at some point in your life. This is a reflection of our hectic life, unbalanced work-life environment, fast food dietary habits, our genes and the climate around us. Everyone is different and attitude plays a big role in stone prevention. However, without changes being made, you have a 50% chance of developing another stone episode within 5 years. General measures in kidney stone prevention revolve around being well motivated to not having another stone, maintaining a healthy lifestyle and normal weight range, keeping up fluid intake, reducing animal protein intake and decreasing salt in the diet.

Is There A Cost Issue
I am not at all sure why the pricing of potassium citrate has become a topic I often hear about from patients, doctors, and just about everybody in the kidney stone world. Certainly the price must have increased, but I cannot find data on the web to prove the point. I also believe Medicare and perhaps other insurers have altered the status of this drug in their payment schedules. Perhaps some of you know more about the problem than I do and are willing to share what you know by way of a comment.
I did find on inspection of the Medicare lists of drug prices by insurance plan that some plans appear to include potassium citrate pills in their formularies at a preferred level and charge as little as $10 for what appears to be 90 pills. Others do not do this and publish higher prices, often as percentages of the retail cash price. Once again, I hope those of you with experiences in purchasing the drug will share what you know.
Baking Soda For Kidney Patients
A daily dose of baking soda could help patients with chronic kidney disease avoid having to undergo dialysis, reported The Times . It said that research has found that sodium bicarbonate can dramatically slow the progress of the condition. The newspaper said that patients given a small daily dose of sodium bicarbonate over a year, had only two-thirds of the decline in kidney function experienced by people given usual care.
This randomised controlled trial found that people with both chronic kidney disease and metabolic acidosis benefited from oral bicarbonate supplements over a two-year period. The study has some shortcomings, but provides strong evidence that these supplements could be used in treatment. The researchers have called for further research to confirm their findings.
The exact place of this in standard treatment for people with chronic kidney disease is not yet known. In practice, people with severe renal disease may receive sodium bicarbonate as part of their treatment in hospital anyway.

What Is A Kidney Stone
A kidney stone is a solid piece of material that forms from crystallization of excreted substances in the urine. The stone may remain in the kidney or break loose and travel down the urinary tract. A small stone may pass all of the way out of the body, but a larger stone can get stuck in the bladder or the urethra. This may block the flow of urine and cause great pain. A kidney stone may be as small as a grain of sand or as large as a pearl, and some are as big as golf balls. Approximately 80 percent of all kidney stones are less than two centimeters in width. They may be smooth or jagged, and are usually yellow or brown in color.

How To Take Apple Cider Vinegar
You can drink ACV if you have kidney stones. It can also be taken regularly as a preventive measure.
Always drink diluted ACV. Undiluted ACV can harm tooth enamel and burn your throat. Rinse your mouth thoroughly after consuming it.
There are several options for taking ACV.
The simplest way to take it is to add 12 tablespoons to a glass of water. Try adding 1 tablespoon of raw honey if you want to make it sweeter.
Its thought that adding lemon juice to ACV can enhance its positive effects on health. This is because lemon juice also contains citrate, which helps to further lower acidity levels in the urine. The citrate can also help prevent new stones from forming and existing stones from becoming bigger. Add 2 ounces of lemon juice to the ACV water.
For whats been called a powerful kidney stone home remedy, combine:
- 12 tablespoons of ACV
- 2 ounces of lemon juice
- 2 ounces of olive oil
Drink this concoction followed by a glass of water.
Another option is to mix 1/2 teaspoon of baking soda and 2 tablespoons of ACV in a glass of warm or lukewarm water. Sodium bicarbonate is the active ingredient in baking soda. It can help alkalize your body and make your urine less acidic. This is thought to help get rid of kidney stones and prevent them from coming back.
You can add ACV to your meals by including it as an ingredient in a host of homemade products, including:
- salad dressings
How Baking Soda Act During Kidney Damage
Sodium bicarbonate is the most common agent used in dialysis or in kidney damage. When sodium bicarbonate is not produced naturally it damages the kidneys. There is a process known as acid buffering helps in removal of waste products and excess fluid is removed from blood. Dialysis helps to add sodium bicarbonate to the body as in kidney failure, kidneys are no longer able to produce it in order to process waste materials and food. The life of kidneys is extended when the body is able to produce normal amount of baking soda in body, the endocrine system can remain healthy automatically.
Sodium bicarbonate is very important for protecting kidneys. Kidneys alone can produce two hundred and fifty grams of sodium bicarbonate per day to neutralize acid in the body.

How To Get Rid Of Kidney Stones Using Baking Soda
August 3, 2016 By Sruthika
Organic substance or mineral salt deposits in the form of solid masses in the kidneys are termed as kidney stones. These stones are crystallized acids that generally filtered through the kidney and pass away from urine.
In some case, the crystallization grows large enough that stuck in the ureter, unable to come out and leads to renal failure. Excessive intake of vitamin D, dehydration, urinary tract infection, mineral imbalance, unhealthy diet, stress, gout, etc. are some of the causes of kidney stones.
Severe pain and tenderness over affected kidney, nausea, blood in the urine, fever, frequent urination, back pain, etc. are some of the symptoms. Men are more prone to these kidney stones but dont panic, as there are many natural ways to get rid of kidney stones rather than over the counter treatments or medications.
Baking soda or sodium bicarbonate is one such effective natural home remedy for treating the kidney stones that dissolve and pass these stones without any pain. It helps to boost up the overall health by balancing the pH levels. Here are the reasons that explain you, how it works for removal of kidney stones.

- A daily dose of sodium bicarbonate — baking soda, already used for baking, cleaning, acid indigestion, sunburn and more — slows the decline of kidney function in some patients with advanced chronic kidney disease, reports an upcoming study.
A daily dose of sodium bicarbonatebaking soda, already used for baking, cleaning, acid indigestion, sunburn, and moreslows the decline of kidney function in some patients with advanced chronic kidney disease , reports an upcoming study in the Journal of the American Society of Nephrology . “This cheap and simple strategy also improves patients’ nutritional status, and has the potential of translating into significant economic, quality of life, and clinical outcome benefits,” comments Magdi Yaqoob, MD .
The study included 134 patients with advanced CKD and low bicarbonate levels, also called metabolic acidosis. One group received a small daily dose of sodium bicarbonate in tablet form, in addition to their usual care. For this group, the rate of decline in kidney function was greatly reducedabout two-thirds slower than in patients. “In fact, in patients taking sodium bicarbonate, the rate of decline in kidney function was similar to the normal age-related decline,” says Yaqoob.
Patients taking sodium bicarbonate also had improvement in several measures of nutrition. Although their sodium levels went up, this didn’t lead to any problems with increased blood pressure.
