Study Design And Population
This is a retrospective cohort study developed at the nephrology ambulatory sector of the Municipal Polyclinic of Divinópolis-MG, Brazil, from March 2017 to March 2019, following the STROBE checklist for observational research. Proton pump inhibitors are amongst the most frequently prescribed medications with an estimated 15 million in the US taking prescription PPIs, making the estimated prevalence around 7.8% of the adult population. This number doesnt even account for over-the-counter usage. Sure, there are some patients that are on pantoprazole for an H. pylori infection, peptic ulcer disease, or severe esophagitis, but then there are those who ended up on it after hospital discharge when they were put on omeprazole for stress ulcer prophylaxis, or were prescribed it as a trial for reflux symptoms, and now are on the medication indefinitely and dont even know it.
PPI use has been linked to several adverse effects including Clostridium difficile infections, vitamin malabsorption, atrophic gastritis, and even dementia and cardiovascular disease. From a kidney perspective, it has long known to be associated with acute interstitial nephritis and hypomagnesemia. However, more recently there are large epidemiologic data that suggest a link between PPI use and CKD progression. This even made major news headlines over the past few years, and most of us have had patients ask us as health care providers if its safe for them to continue taking their Protonix.

Taking Gaviscon with painkillers
It is safe to take painkillers such as paracetamol or co-codamol with Gaviscon.
It’s also fine to take occasional painkilling doses of ibuprofen with Gavsicon. However, regular use of non-steroidal anti-inflammatory drugs such as ibuprofen or aspirin can irritate the lining of the stomach, which may make the symptoms of acid reflux worse.
Gaviscon with other medicines
Gaviscon may reduce the absorption of other medicines from the gut, which could make them less effective.
Gaviscon should preferably not be taken at the same time of day as any other medicines. In general, any interactions can be avoided if you don’t take Gaviscon in the two hours before and after taking any other medicine.

But a new study indicates that more than half of patients who develop chronic kidney damage while taking the drugs don’t experience acute kidney problems beforehand, according to researchers.
Taking popular heartburn drugs for prolonged periods has been linked to serious kidney problems, including kidney failure. The sudden onset of kidney problems often serves as a red flag for doctors to discontinue their patients’ use of so-called proton pump inhibitors , which are sold under the brand names Prevacid, Prilosec, Nexium and Protonix, among others.
But a new study evaluating the use of PPIs in 125,000 patients indicates that more than half of patients who develop chronic kidney damage while taking the drugs don’t experience acute kidney problems beforehand, meaning patients may not be aware of a decline in kidney function, according to researchers at Washington University School of Medicine in St. Louis and the Veterans Affairs St. Louis Health Care System. Therefore, people who take PPIs, and their doctors, should be more vigilant in monitoring use of these medications.

Drinking water during the later stages of digestion can reduce acidity and GERD symptoms. Often, there are pockets of high acidity, between a pH or 1 and 2, just below the esophagus.
