The drugs, called proton pump inhibitors , reduce the symptoms of heartburn by lowering the amount of acid in the stomach. They include the brand names Losec, Nexium, and Prevacid.
Doctors previously monitored patients for acute kidney problems, such as decreased urination, swelling in the legs, ankles, or feet, and nausea. Such symptoms were thought to be a warning of more permanent kidney damage, and would often result in doctors taking patients off the drugs.
But the latest research, in the journal Kidney International, shows this isnt always the case.
What Are Proton Pump Inhibitors
Proton pump inhibitors are medications that help to ease the symptoms of acid reflux. These medications work by blocking enzyme-producing acids and reducing the production of acid in the stomach. These drugs are available in both prescription and over the counter form. Omeprazole is sometimes used in combination with antibiotics to treat ulcers linked to an infection by the H. Pylori bacteria.
When the stomach over-produces acids, ulcers and sores may form in the esophagus, stomach, and duodenum. These problems can present themselves in the form of symptoms like heartburn, persistent coughing, and difficulty swallowing. Patients who reduce their stomach acid through the use of PPIs like Prilosecallow these sores time to heal. Other common PPI drugs are Nexium and Prevacid .

Ppis And Ckd Progression
This one is a bit tougher to nail down, but several large cohort studies have suggested that PPI use is associated with CKD progression. Meta-analyses also support this association, and interestingly, the mechanism is not thought to be related to interstitial nephritis, but rather a direct pathway leading to indolent damage. The biologic mechanism for this chronic damage is unclear, but hypotheses include alteration to the gut microbiome, impairment of tubule regeneration, or upregulation of heme oxygenase-1.
Its important to note that these studies linking adverse kidney outcomes to PPI use are observational in nature and have not been validated in randomized controlled trials . However, due to the low incidence and slow progression of CKD, no RCTs involving PPIs have been powered sufficiently to detect adverse kidney outcomes. Nonetheless, the observational cohort data does allow us as providers to examine the risk/benefit ratio of this medication, and to consider deprescription.

Ppis And Acute Interstitial Nephritis
Weve all been taught this and have likely passed this association on to trainees over the years. Although AIN is not a common phenomenon, the high prevalence of the use of PPI medications brings this association closer to the surface. Because of how often these medicines are used, the exact incidence is tough to pin down, but some studies suggest that this class may be the leading cause of drug-induced AIN. In a single-center UK study of 24 biopsy-confirmed cases of tubulointerstitial nephritis, 14 cases were thought to be drug-induced, and 8 of 14 were presumed to be related to omeprazole or lansoprazole. Other studies have consistently reproduced this association as well. For the small subset of patients that experience AIN from this class of medications, it should be listed as a true allergy and they should not be re-challenged. If continued acid suppression therapy is needed, H2 blockers can be considered. Heres a case I had from a few years back in a patient we biopsied for AKI who was taking a PPI:

What Is The Evidence Linking Ppis And Kidney Damage
Over the past few years, several studies have confirmed that chronic PPI users may face an increased risk of developing kidney damage.
A massive study published in 2018 found that long-term users of PPIs face an increased risk of developing acute kidney injury and chronic kidney disease . A follow-up study found that PPI users were 22% more likely to develop kidney injuries or kidney disease than patients taking H2 blockers.
Several recent studies have also found that these heartburn drugs can lead to a condition known as acute interstitial nephritis , which causes swelling inside of the kidneys. Although patients can be treated without permanent damage to their kidneys if the condition is caught early, AIN can cause serious health problems and permanent kidney damage if left untreated.
In 2014, the Food and Drug Administration required the manufacturers of PPIs like Nexium, Prevacid, and Prilosec to add a warning about the risk of AIN and other kidney problems linked to these medications. Although the agency warned that AIN could develop at any point during PPI therapy, the FDA did not add a black box warning or issue a recall notices as a result of the risk of kidney damage associated with PPIs
Exposure Variable: Omeprazole Use
Among the PPI drugs available, only omeprazole was used by the patients evaluated in this study . This can be justified because it is the only PPI available in the public pharmacies of Divinópolis-MG.
Regular users of omeprazole were considered those patients who had this medicine registered in the medical records for a period of three months or more during the data collection period. Such classification of the participants as current PPI users when the time of use was three months or more was based on the consultation of previous studies . Initially, users of omeprazole were considered as those who at baseline had used this drug previously for at least three months. Thereafter all patients defined as users were evaluated for the other times for the use of omeprazole, and only those who maintained its use during all evaluation times for at least two years were included. Follow-up data on drug use were available for all follow-up times. Only patients who continued to use the drug during all assessment times were considered exposed to omeprazole. In the two years of follow-up, there was no change in the prescription for any of the patients included, maintaining the daily dose of 20 mg/ day. Those who discontinued omeprazole during follow-up were not included in the study.

End Stage Renal Disease
ESRD is of particular concern due to the limited prognosis in the absence of receiving dialysis or a kidney transplant. Very large OR values were determined for three widely used PPIs: omeprazole , esomeprazole , and lansoprazole demonstrating a significant association with ESRD. The frequency of ESRD with pantoprazole monotherapy did not reach statistical significance . Patients who received rabeprazole did not have any ESRD reports .
Outcome Variable: Ckd Stage Evolution
In this study only patients with confirmed diagnosis of CKD were included, patients with acute kidney injury and other renal diseases were not evaluated.
The evaluation of CKD stage and the identification of omeprazole users and non-users occurred at the time of diagnosis in the ambulatory clinic, followed by subsequent consultations. For a large number of patients, the CKD stage was available and recorded by the prescriber in the medical record at the time of the consultation. When the CKD stage was not available, the eGFR was calculated and classified according to the KDIGO and MS guidelines using the CKD-EPI equation without correction for race .
