When Surgery Is Necessary

If you think you might have a kidney stone, you should see your doctor as soon as possible. If youre found to have one, your doctor can help you determine whether to try to pass the stone naturally, take medication, or get the stone surgically removed.

In some circumstances, your doctor might recommend immediate surgical removal without a waiting period. This will usually be because the stone is too big to pass naturally or is blocking urine flow. If the stone is blocking the flow of urine, it can lead to an infection or renal damage.

In other circumstances, your doctor might recommend waiting to see if you can pass the stone on your own. You should check in with your doctor often during this time to see if anything is changing, especially if you have new symptoms.

During the waiting period, your doctor might recommend surgery if the stone continues to grow, youre having unmanageable pain, or you develop signs of infection, such as a fever. Infection, fever, kidney damage, intractable pain, or intractable vomiting are all indications for immediate surgery.

Treatment Of Kidney Stones

For smaller kidney stones, pain relievers may be the only treatment needed. On average it takes five to seven days to pass a kidney stone, says Dr. Abromowitz. It may pass sooner. And if the stone is very high in the ureter, it can take up to two weeks.

Larger stones that block urine flow or cause infection may require surgery, such as:;;

  • Shock-wave lithotripsy, a noninvasive procedure using high-energy sound waves to break stones into fragments that pass out in the urine
  • Ureteroscopy, in which an endoscope is inserted through the ureter to retrieve or break up the stone;
  • Percutaneous nephrolithotomy or nephrolithotripsy, used for very large or irregularly shaped stones. For both procedures, a small incision is made in the back to provide access for a nephroscope, a miniature fiberoptic camera, and other small instruments. Your doctor then either removes the stone or breaks up and removes the stone .;

For ongoingprevention of recurring kidney stones, your doctor may prescribe increasing fluid intake, changing diet, controlling weight, and taking medication.

How To Prepare For Kidney Stone Surgery

When we meet with you, we will give you specific instructions to prepare for your surgery. They may include the following.

  • Stop smoking, if you smoke, well in advance of surgery.
  • Stop taking certain medications that can make it hard for your blood to clot, such as blood thinners like Coumadin, Plavix, Xarellto, and the like.

Dietary modifications: Patients are often encouraged to increase liquid intake throughout the day, reduce sodium consumption and non-dairy animal protein, aiming at 1,000 to 1,200 mg of dietary calcium per day. Causes Of Kidney Stones

The most common cause of kidney stones isdehydration. Youre not drinking enough water to dilute the concentration of minerals in your urine .;Recommended water consumption is about two liters or half a gallon of water a day.

Other kidney stone risk factors include:

  • Too little or too much exercise

How Does Flomax Work For Kidney Stones

Flomax is an alpha-blocker, and in the urinary tract, there are various forms of alpha-adrenergic receptors. These receptors link to smooth muscle. When they become activated would trigger muscle contraction.

This leads to a relaxation of the smooth muscle, that limits not only to the prostatic urethra but the rest of the urinary tract as well. This effect is more pronounced in the distal portion of the ureters and the bladder neck, areas with a higher concentration of alpha 1D receptors .;

When kidney stones are working their way through the ureters, the smooth muscle typically contracts. This can lead to a risk of entrapment of the stone and limiting its progression through the urinary tract .

In these cases, Flomax mediates relaxation of smooth muscles in the ureters. This, in turn, improves the flow of urine and solves the obstruction triggered by kidney stones and aggravated by smooth muscle contraction.

This effect by tamsulosin would also reduce the consequences of mechanical obstruction in the urinary tract. Such as hydronephrosis . And complete urinary obstruction .;

I have 2 x 5mm kidney stones, one on each side. I was first put on Doxazosin, but that did not pass either of them so my doctor switched me to Flomax. I just took my second dose but I was wondering if anyone knew about how long it will take to pass them? I have only had random pains, nothing severe enough to say it is moving. I also have H. Pylori so nausea is a given. I just have so much going on medically it would be nice to get these out of the way.

4 days on Flomax and still no pain so I am guessing no movement. Can a 5mm stone get stuck in the kidney and never come out?

kaismama

Drink lots and lots of fluids. Wash them down. I don’t envy you if they both go at the same time.

I have been drinking so much water ugh. I am not able to drink anything else because of the H. Pylori so it is my only option.

I understand that Flomax simply relaxes the ureters which allows the stones to pass thru easier, it will not accelerate the passing of the stones. Love the comment about being a kidney stone making machine, it made me chuckle, especially since I learned today I have 9, count them, 9 stones in EACH kidney, for a grand total of 18 right now, all roughly 5 & 6mm in size. I’ve got some fun days ahead.

How Are Kidney Stones Treated

Once diagnosed, your healthcare provider will first determine if you even need treatment. Some smaller kidney stones may leave your system when you urinate. This can be very painful. If your provider decides that you do need treatment, your options include medications and surgery.

Medications. Medications may be prescribed to:

  • Your healthcare provider may recommend that you take an over-the-counter medication like ibuprofen or, if youre in the emergency room, an IV narcotic.
  • Manage nausea/vomiting.
  • Relax your ureter so that the stones pass. Commonly prescribed medicines include tamsulosin and nifedipine .

You should ask your healthcare provider before you take ibuprofen. This drug can increase the risk of kidney failure if taken while youre having an acute attack of kidney stones especially in those who have a history of kidney disease and associated illnesses such as diabetes, hypertension and obesity.

Surgery. There are four types of surgeries used to treat kidney stones. The first three are minimally invasive, meaning that the surgeon enters your body through a natural opening , or makes a small incision.

How Effective Is Tamsulosin For Kidney Stones

When given tamsulosin for kidney stones, the majority of men are able to quickly eliminate the stones and return to normal health. This medication is most effective when used alongside another treatment that breaks up larger stones into small pieces. In women, taking tamsulosin is not effective for treating kidney stones.

Recent medical studies have confirmed that giving male patients tamsulosin can effectively treat kidney stones. This drug works well when combined with extracorporeal shock wave lithotripsy , a treatment that uses high-intensity sound waves for breaking kidney stones into smaller pieces. Once the stones are broken up, they are able to pass out of the body in urine. Though ESWL can be used alone, nearly half of patients who receive only this treatment need to have it more than once. Only about a quarter of patients who receive ESWL without the administration of tamsulosin see the condition clear up within a month and only about half within three months.
