How Is Hsp Treated
There is no specific treatment for HSP. The main goals of treatment are to relieve symptoms such as joint pain, abdominal pain, or swelling. In most cases, you can use over-the-counter medicines, such as acetaminophen , for the pain. In some patients with severe arthritis, the doctor may prescribe prednisone, a steroid medicine. As mentioned earlier, the rash and joint symptoms usually go away after 4 to 6 weeks without causing permanent damage. Severe problems with the bowels are rare in HSP, especially in younger children. If you have severe pain or severe bleeding in the digestive tract your doctor may prescribe prednisone, or the problem may need to be corrected with surgery. Your doctor will check your kidney function with blood and urine tests even after the main symptoms of HSP disappear. People who develop kidney disease usually show signs within 3 to 6 months after the initial rash appears. If signs of kidney disease appear, your doctor will refer you to a nephrologist, who may prescribe drugs to suppress the immune system. These immunosuppressive drugs may keep kidney disease from progressing to permanent kidney failure.
Pigmentary Changes Or Skin Discoloration
Many reported cases of color change in skins, or hyperpigmentation, happen to people with ESRD. One main reason for skin discoloration is associated with pigments called urochromes and retained in the skin.
Usually, it gets excreted when you have healthy kidneys. Most patients with such condition tend to have a greyish, almost metallic like color skin conditions in kidney failure.
Another discoloration is called uremic frost. It is a white, powdery substance left on the skin surface after the sweat dries. Uremic frost is getting prevented by getting adequate dialysis.
What Is The Long
Most cases of HSP resolve within 4 to 6 weeks without long-term problems. About one in three people has a recurrence of HSP. Recurrences usually occur within a few months and are usually less severe than the initial episode. When the symptoms recur or last longer than 6 weeks, they can be very frustrating and uncomfortable. The long-term outlook is still good, however, as long as your kidneys are healthy. If you develop progressive kidney disease, you will need to have regular checkups to monitor your kidney function. In the early stages of kidney disease, you may not have any symptoms, but blood and urine tests may show that your kidney function is declining. If you continue to have blood and protein in your urine, you are at greater risk of developing chronic kidney disease. Between 20 and 50 percent of children with HSP develop some kidney problems, but only 1 percent progress to total kidney failure. Progression to kidney failure may take as long as 10 years.

What Else Should I Know
Most children with HSP fully recover within a month and have no long-term problems. Kids whose kidneys are affected will need to see a doctor for regular checkups to monitor kidney function.
Some kids who have HSP get it again, usually a few months after the first episode. If it does come back, it’s usually less severe than the first episode.
Dehydration And Dry Skin
People with ESRD often have dry skin, but the connection between dry skin and itching is not so clear.; What has been determined is that dry skin makes the itching worse and more severe.;
Getting less fluid can cause itchy skin to get worse. Hydration is important for kidney health especially if you are on dialysis. The fluid removed during dialysis needs to be replenished. So getting enough hydration between treatments could help lessen the intensity of the itch.;
Dry skin becomes dangerous when it turns into nephrogenic systemic fibrosis . This condition results in the hardening of the skin causing it to become thick and tight. Some people with kidney disease also have nerve damage that prevents them from sweating enough. This could lead to dry skin and is another reason to keep hydrated.;

Physical Therapies For Itching In Kidney Disease
There are therapies that can be done to the skin itself. These physical therapies include phototherapy and acupuncture.
Itching in Kidney Disease
Ultraviolet B phototherapy works by stopping the spread of the substances that cause the itch. This therapy also stops inflammation from the immune response.
UVB phototherapy is effective for ESRD pruritus. Some therapies perform better than others.;
Narrow band-UVB phototherapy and UVA alone do not appear to be as effective. Itching came back soon after 6 weeks of NB-UVB. To fully control itching, these therapies need to be done continuously over time. Initially, this type of phototherapy needs to be done three times per week. After that each week an additional one to two sessions needs to be done for maintenance.;
Broadband phototherapy worked the best. Nine out of 10 patients had a marked improvement in itching after 6 BB-UVB treatments. These effects lasted for at least six months.

Newer Therapies For Itching In Kidney Disease
Uremic pruritus could not just be an isolated skin condition and may have underlying neurological ramifications that need to be addressed. The most likely scenario is that there is an imbalance in the nervous system. The answer to the itch would be correcting these imbalances.
Opiates trigger itching so finding an opiate antagonist may be the solution to this itchy problem. When tested these medications had some success. The best medication for stopping itch was that consisting of both an agonist and antagonist.
Known as opioid receptor agonist-antagonists, these compounds are thought to be more effective than pure agonists. These medications also alleviated itch as well as may inhibit scratching.

Nalfurafine
