NEW YORK Certain diet sodas may have the potential to prevent the most common type of kidney stone, if new lab research is correct.

In the study, researchers found that the diet versions of several popular citrus-flavored sodas like 7Up, Sunkist and Sprite contained relatively high amounts of a compound called citrate. Citrate, in turn, is known to inhibit the formation of calcium oxalate stones, the most common form of kidney stone.

The findings, reported in the Journal of Urology, suggest that diet sodas could stand as an extra weapon for some people prone to forming kidney stones.

Kidney stones develop when the urine contains more crystal-forming substances such as calcium, uric acid and a compound called oxalate than can be diluted by the available fluid. Most kidney stones are calcium-based, usually in combination with oxalate.

One reason that certain people are prone to being stone-formers is that their urine contains relatively little citrate, explained Dr. Brian H. Eisner, a urologist at Massachusetts General Hospital in Boston and the lead researcher on the new study.

Women who drank two or more diet sodas a day had a 30% drop in a measure of kidney function during the lengthy study follow-up, according to research presented Saturday at the annual meeting of the American Society of Nephrology in San Diego.

Thirty percent is considered significant, says researcher Julie Lin, MD, MPH, assistant professor of medicine at Harvard Medical School and a staff physician at Brigham and Womens Hospital in Boston. Thats especially true, she says, because most study participants had well-preserved kidney function at the start of the study.

Increased alkalinity is proven to augment citraturia, a known factor for calcium stones. Malate increases the amount of alkali delivered. Researchers measured the citrate and malate content of 15 popular diet sodas. The researchers found that Diet Sunkist Orange contained the greatest amount of total alkali and Diet 7-Up had the greatest amount of citrate as alkali.

“This study by no means suggests that patients with recurrent kidney stones should trade in their water bottles for soda cans,” said Anthony Y. Smith, MD, an AUA spokesman. “However, this study suggests instead that patients with stone disease who do not drink soda may benefit from moderate consumption.”

The study was presented at the 104th Annual Scientific Meeting of the American Urological Association .

In fact, compared with women who did not drink diet soda, soda-drinking women had a 30% greater reduction in kidney function in 20 years. More than 3,000 women have participated in the study for 20 years or more now the women’s median age is 67 years.

At the start of the study, all the women had healthy kidney function, according to Julie Lin, MD, MPH, assistant professor of medicine at Harvard Medical School and a physician at Brigham and Women’s Hospital.

Drinking one diet soda daily did not decrease kidney function more than normal. Drinking two or more diet sodas, though, appeared to cause problems.

With some 37 million Americans suffering from chronic kidney disease, it is a major public health problem. The study with the nurses will have to be confirmed by other studies. But in the meantime, water may be the best thing to drink.
