Sodium Intake Affects Urine Calcium Response To Acid
In a prior article I showed the urine calcium lowering effects of potassium citrate were independent of the effects of diet sodium. But those data, and trial data like it are observations. Suppose you give people an acid load so their urine calcium goes up as a result of what you did and also varied sodium intake a direct experiment, not observations.
I did that, and although others also may have done the same, my experiment was a good one and I like it.
Four people were studied during three control days points to the left on the graph. They had normal urine calcium excretions , normal serum PTH levels and normal serum total calcium and ionized calcium . That is what one expects from normal people.
I gave them ammonium chloride which is an acid load, and as you might expect by now their urine calcium should go up. But, I also lowered their sodium intake to 40 80 mEq daily and take a look urine calcium did not change.
Their blood became distinctly acidic you will have to look at the paper. The calcium ion rose and the total calcium fell because the acidity tends to liberate calcium ion from binding to blood proteins and from phosphate complexes.
I then raised the diet sodium to about 200 mEq and there was the urine calcium increase. Serum PTH went up, too, perhaps because the kidneys were losing calcium.

How Many Grams Of Protein A Day Is Too Much
If there is a level of protein intake that is excessive and damaging to kidney health, we dont currently have evidence to show what this may be.
In other words, there is currently no such thing as too much protein regarding negatively impacting renal function.
Studies show that protein intake at up to 35% of total energy intake has no adverse effect on people with healthy kidneys.
The four studies mentioned in this article also consider all different people.
From resistance-trained individuals to pre-diabetics, people with abdominal obesity, and average healthy adults, no harm was found from increasing protein intake.
Of course, we should still be sensible, and we shouldnt overeat protein just for the sake of it. But overall, the data suggest that there would be no harm from consuming around 1 gram protein per pound of body weight.
Consuming more protein than this would likely be unnecessary for the majority of people.

There have been studies done to prove or disprove the idea of whey protein being able to cause kidney stones, so whats the truth?
How Much Protein Do We Need
A subsequent WHO meta analysis of mostly the same underlying data supplemented by more recent studies comes to much the same conclusions, but in perhaps a more nuanced manner. A more recent analytical critique of the whole matter is not remarkably far off in estimates for adults, though pregnancy and childhood seem controversial.
This summary graph from the critique gives a sense of how the protein requirements are set. The median requirement is where about half of all studied subjects were in neutral nitrogen balance their body protein mass would be stable, a very important matter. The safe population intake is set higher. The safe individual value is high enough enough that 97.5% of the individuals in a population would be in balance: almost all people would not lose protein mass consuming this amount of protein for example muscle. WHEN HEALTHY HABITS MAY CAUSE KIDNEY STONES
Consuming too much of the bad stuff such as meat, salt and soft drinks can increase your risk.
But certain fruits and vegetables, too? So, which is the more likely cause of kidney stones?
The most common causes of kidney stones in Singapore are related to insufficient fluid intake, too much salt in the diet and too much protein, said Dr Tan.
Still, it is interesting to note that too much of a good thing can be bad for you.
Here are some examples of healthy habits or intentions that may increase your risk of or exacerbate your kidney stones:
- YOU POP VITAMIN C LIKE CANDY
Too much Vitamin C can be an issue as it combines with calcium to form calcium oxalate a precursor to certain kinds of kidney stones.
Vitamin C in high doses is a problem, said Dr Tan.
And by that, he means doses of more than 500mg per day. In fact, I would rather my patients keep to a maximum of 200mg per day, he said.
- YOU TAKE CALCIUM SUPPLEMENTS
Calcium supplementation in large quantities, and when taken separately from meals, may set you up for kidney stones, according to a study published in the Translational Andrology And Urology journal.
But calcium is necessary for strong bones, and it is lacking in Singaporeans’ diets, said Dr Tan.
Furthermore, although it sounds paradoxical, calcium can also prevent kidney stones.
But if boosting your calcium intake doesnt lower your urine oxalate level, the same programme suggested watching your oxalate intake.
Chronic Kidney Disease And Diet
From 1990 to 2010, the incidence of Chronic Kidney Disease has doubled. In a review of the standard American diet and its effects on the kidney, the diet appears to have negative effects on human renal function. A decline in renal function was observed in those who consumed an animal protein-rich diet, as compared to those consuming a plant protein-rich diet. Consumption of excessive animal proteins leads to a marked acid load to the kidney, and has been associated with the development of kidney stones. Higher meat consumption leads to increased renal acid excretion and the production of ammonia, which can cause metabolic acidosis and a higher risk for tubulointerstitial injury. In addition to eliminating meat, it is beneficial to add dietary fiber. In a meta-analysis of 14 trials involving 143 participants, the additional dietary fiber significantly reduced serum urea and creatinine levels.
Consumption of animal products impairs renal vascular function, leading to increased inflammation and subsequent microalbuminuria. Studies show that meat ingestion impacts factors such as GFR, glucagon, prostaglandins, and increased albuminuric response. Glomerular hyperfiltration is thought to be due to an inflammatory response via vasodilatory prostaglandins, as a result of animal protein consumption. This is supported by studies showing that the level of hyperfiltration decreases when indomethacin is administered.
