The study, which involved over 250,000 people, found the use of PPIs was associated with a 20 to 50 percent higher risk of chronic kidney disease . While those behind the study are clear to point out that it doesnt directly prove that PPIs cause CKD due to the fact that the study was designed to determine correlation and not causation the results show a frightening risk for those taking these medications.

Researchers expressed concern about the frequency at which doctors prescribe these medicines, noting that PPIs are being prescribed for conditions like mild heartburn when the FDA recommends them only for more serious gastrointestinal disorders. One researcher pointed out that up to 90 percent of people taking PPIs dont exhibit indications or symptoms specified by the FDA. The takeaway is that not only do doctors need to be more cautious when prescribing or suggesting PPIs, but they should also be upfront with patients about the risks involved with taking these medications.

Prilosec Other Heartburn Meds Linked To Kidney Failure In The Elderly

Older patients taking proton pump inhibitors, a common remedy for heartburn and acid reflux, are two times more likely to be hospitalized with kidney failure than peers who don’t take the pills, a study finds.

While the side effect is extremely rare, and the study doesn’t prove the drugs cause kidney failure, the association is worrisome because tens of millions of people a year take these pills, sold by prescription and over-the-counter in some countries, with brand names including Prilosec, Prevacid and Zegerid.

“Generally, the drugs are very well tolerated, and the vast majority of patients who take them will not develop or other serious problems,” lead study author Tony Antoniou, a researcher at the Institute for Clinical Evaluative Sciences and St. Michael’s Hospital in Toronto, said by email. “But the drugs should be used for the shortest possible duration.”

Antoniou and colleagues identified about 290,000 people aged 66 or older taking proton pump inhibitors in Ontario between 2002 and 2011, as well as a similar number of individuals not taking the drugs.

The researchers then looked at how many people were hospitalized for acute kidney injury within 120 days of starting treatment, and compared that to the group not taking the drugs.

What Are Proton Pump Inhibitors

Proton pump inhibitors are medications that help to ease the symptoms of acid reflux. These medications work by blocking enzyme-producing acids and reducing the production of acid in the stomach. These drugs are available in both prescription and over the counter form. Omeprazole is sometimes used in combination with antibiotics to treat ulcers linked to an infection by the H. Pylori bacteria.

When the stomach over-produces acids, ulcers and sores may form in the esophagus, stomach, and duodenum. These problems can present themselves in the form of symptoms like heartburn, persistent coughing, and difficulty swallowing. Patients who reduce their stomach acid through the use of PPIs like Prilosecallow these sores time to heal. Other common PPI drugs are Nexium and Prevacid .

In the United States, PPI drugs are some of the most commonly used medications. In fact, about 21 million Americans were prescribed PPI drugs in 2009. Now, about 10 percent of American adults take some kind of PPI medication.

Weve all been taught this and have likely passed this association on to trainees over the years. Although AIN is not a common phenomenon, the high prevalence of the use of PPI medications brings this association closer to the surface. Because of how often these medicines are used, the exact incidence is tough to pin down, but some studies suggest that this class may be the leading cause of drug-induced AIN. In a single-center UK study of 24 biopsy-confirmed cases of tubulointerstitial nephritis, 14 cases were thought to be drug-induced, and 8 of 14 were presumed to be related to omeprazole or lansoprazole. Other studies have consistently reproduced this association as well. For the small subset of patients that experience AIN from this class of medications, it should be listed as a true allergy and they should not be re-challenged. If continued acid suppression therapy is needed, H2 blockers can be considered. Heres a case I had from a few years back in a patient we biopsied for AKI who was taking a PPI:

Database Design And Processing

After all data were collected, a database was constructed and filled using Epidata software version 3.1. Data were double typed. The Kappa agreement test was applied to validate the database, resulting in a Kappa coefficient of 0.92, reflecting an almost perfect agreement . What Is Chronic Kidney Disease

What Is The Link Between Ppis And Kidney Failure

In some cases, the kidney damage caused by PPIs can be very serious. In 2019, researchers at the University at Buffalo School of Pharmacy and Pharmaceutical Sciences published a study which found that PPI users were 20% more likely to develop chronic kidney disease and four times more likely to develop acute kidney injury than non-users.

If left untreated, these problems can lead to kidney failure in some patients. Kidney failure occurs when the kidneys can no longer effectively remove waste products from the body. Patients who develop kidney failure may need dialysis or a kidney transplant in order to keep them alive.

Even more troubling is the fact that many of these patients show no signs of kidney problems before the onset of renal failure. According to a 2017 study by scientists at the Washington University School of Medicine, the kidney problems caused by PPIs can develop silently and gradually over time, eroding kidney function and leading to long-term kidney damage or even renal failure. The study found that more than 50% of PPI users who developed kidney failure or chronic kidney disease showed no symptoms before they were diagnosed.

Study Design And Population

This is a retrospective cohort study developed at the nephrology ambulatory sector of the Municipal Polyclinic of Divinópolis-MG, Brazil, from March 2017 to March 2019, following the STROBE checklist for observational research. Eligibility criteria were defined as all adult and elderly patients of both genders with established CKD diagnosis who were already undergoing clinical follow-up or who were diagnosed after insertion in the nephrology ambulatory clinic in the years 2016 and 2017. During this period, 241 patients were attended. Of these, 42 patients were excluded due to lack of confirmation of CKD diagnosis , absent creatinine values and history of neoplasms . Patients without baseline creatinine values were excluded from the study because it would not be possible to determine the estimated glomerular filtration rate and thus the baseline CKD stage. Similarly, those patients who had not recorded serum creatinine values at subsequent times were also excluded. Patients who underwent previous kidney transplantation and/or patients on renal replacement therapy, and patients who had missing data related to some of the variables used during the two-year follow-up were not included in this study. Thus, a total of 199 patients with CKD eligible for the study were included.

A study that examined the use of common heartburn medications in a large patient population showed ties between the medications and CKD and acute kidney failure later in life.

University at Buffalo researchers reviewed the medical records of 190,000 patients older than 15 years of age and found acid reflux drugs increased the risk of kidney disease by 20% and the risk of kidney failure by four times compared to individuals unexposed to the drugs. Risks were highest among people at least 65 years old, the researchers said.

Proton pump inhibitors have been linked previously with causing kidney disease, including AKI.

What Are The Side Effects Of Ppis

Although they are among the most widely prescribed medications in the U.S., researchers have linked PPIs to an increased risk of serious and potentially deadly side effects.

One of the most dangerous side effects linked to the use of PPI medications is kidney disease. Studies have shown that long-term PPI users are more likely than patients taking H2 blockers to develop problems with their kidneys, including kidney damage and kidney failure.

Other side effects that have been linked to the use of PPIs include an increased risk of fractures, increased pneumonia risk, an increased risk of Clostridium difficile infections, iron and Vitamin B12 deficiency, and interactions with some other prescription drugs.

Omeprazole May Worsen The Progression Of Chronic Kidney Disease

The use of PPIs, in particular omeprazole, has also been associated with the development of CKD. It is thought that this may be due to omeprazole causing oxidative stress and renal tubular cell death.

In one study following 199 patients with CKD for a year, 70.6 percent of patients using omeprazole had disease progression compared with only 10.5 percent of patients who did not take omeprazole.

An analysis of Food and Drug Administration post marketing surveillance data has also shown that people taking PPIs are 28.4 times more likely to report CKD and 35.5 times more likely to report end-stage renal disease than people taking histamine-2 receptor antagonists . H2RAs, or H2 blockers, are another class of drug used to reduce the amount of acid produced in the stomach, but they are not considered to work as well as PPIs. Examples of H2 blockers include nizatidine , famotidine and cimetidine .

A study published in the journal Nutrients discussed a link between PPIs and kidney stones.

The authors note:

Within the PPI cohort, patients who received the following PPIs as monotherapy had a significant increase in the frequency of nephrolithiasis reports: omeprazole , esomeprazole , pantoprazole , and lansoprazole .
