Sometimes people with kidney cancer do not have blood in their urine until the cancer is quite large and might have spread to other parts of the body.

Imaging tests such as computed tomography scans and magnetic resonance imaging scans can often find small kidney cancers, but these tests are expensive. Ultrasound is less expensive and can also detect some early kidney cancers. One problem with these tests is that they cant always tell benign tumors from small renal cell carcinomas.

Often, kidney cancers are found by accident during imaging tests for some other illness or symptom. These cancers usually are not causing pain or other symptoms when they are found. The survival rate for these kidney cancers is very high because they are usually found at a very early stage.

Relieving Symptoms And Problems Caused By Ckd

If CKD becomes severe you may need treatment to combat various problems caused by the poor kidney function. For example:

  • Anaemia may develop which may need treatment with iron or erythropoietin – a hormone normally made by the kidneys.
  • Abnormal levels of calcium or phosphate in the blood may need treatment.
  • You may be advised about how much fluid to drink, and how much salt to take.
  • Other dietary advice may be given which can help to control factors such as the level of calcium and potassium in your body.

If end-stage kidney failure develops, you are likely to need kidney dialysis or a kidney transplant to survive.

People with stage 3 CKD or worse should be immunised against influenza each year, and have a one-off immunisation against pneumococcus. People with stage 4 CKD should be immunised against hepatitis B.

Is Kidney Failure Permanent

Usually, but not always. Some kinds of acute kidney failure, also known as acute renal failure, get better after treatment. In some cases of acute kidney failure, dialysis may only be needed for a short time until the kidneys get better.

In chronic or end stage kidney failure, your kidneys do not get better and you will need dialysis for the rest of your life. If your doctor says you are a candidate, you may choose to be placed on a waiting list for a new kidney.

What If I Have Chronic Kidney Disease Stage 3

Stage 3 means moderate CKD. This often means that you do not need to see a kidney specialist but your GP will need to see your regularly for monitoring, including blood and urine tests.

You may need treatments to reduce your risk of progressing to more severe CKD. You will probably also need other treatments to reduce your risk of any other problems, particularly cardiovascular diseases .

However if regular blood and urine tests show that your CKD is progressing to stage 4 then you will usually need to be referred to a kidney specialist to consider further assessments and treatment.

Potential Changes To Ckd Staging

KDIGO has recently formed an international work group to update and revise the current CKD staging system. The deadline for publication of the updated guidelines is sometime in 2012. Potential changes to the current system include the addition of a measure of proteinuria in all stages of CKD, including a consideration of the cause of CKD, splitting stage 3 into 3a and 3b, and recommending the use of the CKD-Epi eGFR equation over the MDRD study equation.

What Causes Chronic Kidney Disease

A number of conditions can cause permanent damage to the kidneys and/or affect the function of the kidneys and lead to CKD. Three common causes in the UK, which probably account for about three in four cases of CKD in adults, are:

  • Diabetes. Diabetic kidney disease is a common complication of diabetes.
  • High blood pressure. Untreated or poorly treated high blood pressure is a major cause of CKD. However, CKD can also cause high blood pressure, as the kidney has a role in blood pressure regulation. About nine out of ten people with CKD stages 3-5 have high blood pressure.
  • Ageing kidneys. There appears to be an age-related decline in kidney function. About half of people aged 75 or more have some degree of CKD. In most of these cases, the CKD does not progress beyond the moderate stage unless other problems of the kidney develop, such as diabetic kidney disease.

Other less common conditions that can cause CKD include:

  • Diseases of the tiny filters , such as inflammation of the glomeruli in the kidneys .
  • Narrowing of the artery taking blood to the kidney

Chronic Kidney Disease Stage 3

Stage 3 of CKD is diagnosed based on estimated glomerular filtration rate readings. This is a blood test that measures creatine levels. An eGFR is used to determine how well your kidneys are working at filtering wastes.

An optimal eGFR is higher than 90, while stage 5 CKD presents itself in an eGFR of less than 15. So the higher your eGFR, the better your estimated kidney function.

Stage 3 CKD has two subtypes based on eGFR readings. You may be diagnosed with stage 3a if your eGFR is between 45 and 59. Stage 3b means your eGFR is between 30 and 44.

The goal with stage 3 CKD is to prevent further kidney function loss. In clinical terms, this can mean preventing an eGFR of between 29 and 15, which indicates stage 4 CKD.

You may not notice symptoms of chronic kidney problems in stages 1 and 2, but the signs start to become more noticeable in stage 3.

Some of the symptoms of CKD stage 3 may include:

  • dark yellow, orange, or red urine
  • urinating more or less frequently than normal

There are several kinds of peritoneal dialysis but two major ones are:Continuous Ambulatory Peritoneal Dialysis and Automated Peritoneal Dialysis .

Continuous Ambulatory Peritoneal Dialysis is the only type of peritoneal dialysis that is done without machines. You do this yourself, usually four or five times a day at home and/or at work. If you are at risk for kidney disease, your provider may check your urine for albumin.

Albumin is a protein found in your blood. A healthy kidney doesnt let albumin pass into the urine. A damaged kidney lets some albumin pass into the urine. The less albumin in your urine, the better. Having albumin in the urine is called albuminuria.

A health care provider can check for albumin in your urine in two ways:

Dipstick test for albumin. A provider uses a urine sample to look for albumin in your urine. You collect the urine sample in a container in a health care providers office or lab. For the test, a provider places a strip of chemically treated paper, called a dipstick, into the urine. The dipstick changes color if albumin is present in the urine.

Urine albumin-to-creatinine ratio . This test measures and compares the amount of albumin with the amount of creatinine in your urine sample. Providers use your UACR to estimate how much albumin would pass into your urine over 24 hours. A urine albumin result of

  • 30 mg/g or less is normal
  • more than 30 mg/g may be a sign of kidney disease

If you have albumin in your urine, your provider may want you to repeat the urine test one or two more times to confirm the results. Talk with your provider about what your specific numbers mean for you.

Half Of All Kidney Cancer Patients Initially Misdiagnosed

Half of all patients with kidney cancer are initially misdiagnosed, data from Kidney Cancer UKs patient survey has revealed.

In 45% of patients surveyed the disease was first mistaken for less serious conditions such as a urine infection, kidney stones or respiratory problems, the charity said.

The survey also revealed that 73% of patients showed no signs of the disease before diagnosis, and in 42.6% it was discovered incidentally, so not related to feeling well or kidney cancer.

The findings are particularly pertinent given that once diagnosed, around half of patients are already at the most advanced form of the disease, stage III or IV, where the prognosis is much worse.

Kidney Cancer UK says that in light of the shocking results and increasing prevalence of the disease it is calling on the government to support research into a national kidney cancer screening programme, to help accelerate diagnosis and effective treatment for patients.

The finding that, of those surveyed, almost half of patients had no symptoms before diagnosis really shows how difficult it is to diagnose kidney cancer and the urgent need for better ways of identifying this silent killer earlier, said GP Dr Juliet Usher-Smith.

We urgently need clear quality standards and performance indicators to support clinicians to help them identify at-risk patients, until a simple test to detect kidney cancer can be developed, added Nick Turkentine, the charitys chief executive.
