What Does Sodium Bicarbonate Do For The Kidneys

Sodium Bicarbonate May Mitigate Calcium Phosphate Stone Risk In Ckd

What is Sodium Bicarbonate?

Sodium bicarbonate, which is commonly used to treat metabolic acidosis in patients with chronic kidney disease , is hypothesized to increase the risk for calcium phosphate kidney stones by increasing urinary pH. New study findings presented at the virtual National Kidney Foundation 2021 Spring Clinical Meetings suggest that although sodium bicarbonate does increase urinary pH, it significantly raises urinary citrate levels, which may mitigate the risk for calcium phosphate stones.

A team led by Kalani Raphael, MD, of Oregon Health & Science University in Portland, and colleagues studied 168 patients with CKD who participated in the randomized BASE Pilot Trial. Of these, 76 received higher-dose and 47 received lower-dose oral sodium bicarbonate, and 45 received placebo.

Compared with placebo, the higher-dose and lower-dose sodium bicarbonate groups experienced a significant mean 0.97 and 0.77 increase in urinary pH from baseline, Dr Raphaels group reported in a poster presentation. Urinary citrate/creatinine concentration increased significantly by a mean 145.7% and 108.3% in the higher-dose and lower-dose sodium bicarbonate groups, respectively. Sodium bicarbonate had no effect on urinary calcium or phosphate.

I dont think these findings should totally alleviate concerns about promoting calcium phosphate stones, Dr Raphael told Renal & Urology News, but we understand some of the chemical changes that happen with sodium bicarbonate a little bit better.

Strengths And Weaknesses In Relation To Other Studies

Given the association between metabolic acidosis and a range of adverse outcomes seen in CKD, it would be expected that amelioration of acidosis could improve physical function, lessen deterioration in renal function and improve measures of cardiovascular health and bone and mineral metabolism in patients with CKD. A recent systematic review found no randomised controlled trial evidence around the effect of bicarbonate on physical function or quality of life the current trial therefore provides an important test of the effect of bicarbonate on these important patient-centred outcomes. A previous single-centre trial of bicarbonate versus usual care found progressive increases in serum bicarbonate over a 2-year period in the bicarbonate group measures of anthropometry were better in the bicarbonate group, and fewer participants progressed to end-stage renal disease in the bicarbonate group . However, this trial did not use placebo, and thus, healthcare providers and participants were aware of group allocation. It is possible that the lack of masking could inflate the effect size seen in this trial, perhaps by an impact on decision-making around renal replacement and on other treatment decisions driven by knowledge of whether a participant was receiving bicarbonate. Participants in this previous trial were recruited from a single centre and were much younger , and a high percentage were of south Asian or black origin.

The Question Of Salt Sensitivity

Salt affects people differently. Some people can consume sodium with no effect on their blood pressure, says Dr. Thomas. But for others who are salt sensitive, even a slight increase in sodium intake wreaks havoc on the kidneys ability to regulate fluid, and increases blood pressure.

Salt sensitivity is most prevalent among people who are middle-aged or elderly, overweight or obese, and African-American. It also tends to become more prevalent as we age.

What Are The Complications Of Metabolic Acidosis If I Have Kidney Disease Or Kidney Failure

  • Increased bone loss : Metabolic acidosis can lead to a loss of bone in your body. This can lead to a higher chance of fractures in important bones like your hips or backbone.
  • Progression of kidney disease: Metabolic acidosis can make your kidney disease worse. Exactly how this happens is not clear. As acid builds up, kidney function lowers and as kidney function lowers, acid builds up. This can lead to the progression of kidney disease.
  • Muscle loss: Albumin is an important protein in your body that helps build and keep muscles healthy. Metabolic acidosis lowers the amount of albumin created in your body, and leads to muscle loss, or what is called muscle wasting.
  • Endocrine disorders: Metabolic acidosis interferes with your bodys ability to maintain normal functions of your endocrine system . This can cause your body to build a resistance to insulin . If left untreated for too long or not corrected in time, it can lead to diabetes.

What Causes Metabolic Acidosis In Chronic Kidney Disease

Healthy kidneys remove acid from the body through urine and they keep the right amount of bicarbonate in the blood. But in CKD, the kidneys canât remove enough acid, which can lead to metabolic acidosis.

The normal level of serum bicarbonate is 22-29 mEq/L. Kidney experts recommend that patients not have their serum bicarbonate levels fall below 22 mEq/L.

What Are Bicarbonate And Serum Bicarbonate

Sodium (salt) and your kidneys

Bicarbonate is a substance called a base, which the body needs to help keep a normal acid-base balance. This balance prevents your body from becoming too acid, which can cause many health problems. The lungs and kidneys keep a normal blood pH by removing excess acid.

Serum bicarbonate mostly travels throughout the body as carbon dioxide , a gas that’s dissolved in the blood. That’s why the amount of CO2 in your blood is used to measure serum bicarbonate , and to therefore check your acid-base balance. If CO2 is too low in your blood, then that means serum bicarbonate is low, and your body has too much acid. Too much acid in the body is called metabolic acidosis.

What Causes Hyponatremia

A low sodium level in your blood may be caused by too much water or fluid in the body. This watering down effect makes the amount of sodium seem low. Low blood sodium can also be due to losing sodium from the body or losing both sodium and fluid from the body.

Hyponatremia can be the result of illnesses and medications. Some causes that may be related to kidney disease include:

  • Kidney failure the kidneys cannot get rid of extra fluid from the body
  • Congestive heart failure excess fluid builds up in the body
  • Diuretics makes the body get rid of more sodium in the urine
  • Antidepressants and pain medication may cause more sweating or urinating than normal
  • Severe vomiting or diarrhea the body loses a lot of fluid and sodium
  • Excessive thirst causes too much fluid intake

Changes In What You Eat

For people with metabolic acidosis, making changes in what they eat may help. For example, eating plant-based protein instead of animal-based protein may keep acid levels lower. Always talk to your doctor before you make any changes to your diet. Your doctor may also recommend that you see a registered dietitian who can help create a specific nutritional plan just for you.

The Adverse Events During The Study Period

Mean body weight was significantly increased after 12 months in the treatment group of stage 5 CKD. There was no significant difference in the changes of body weight during study period between the treatment group and control group . In stage 4 CKD, there was no statistically significant difference in the change of body weight during the study between the treatment group and control group. However, in stage 5 CKD, there was a significant difference in the changes of body weight during the study between the treatment group and control group .

There was no significant difference in the changes of systolic and diastolic blood pressure during the study between the treatment group and control group . There was no difference in the number and types of antihypertensive medications during the study between the two groups. All patients in stage 5 CKD was received loop diuretics at the end of the study. Loop diuretic use increased similarly by 75 and 82 % in the control and treatment group, respectively. Three patients in the treatment group and 4 patients in the control group required dialysis during the study period.

Baking Soda Dose For Kidneys Infections & Stones

In this section, I will highlight the dosage of sodium bicarbonate for kidney diseases, infections, and stones. Kidneys are critical organs in our bodies.

While they may be considerably smaller, they play a crucial role in ensuring that our lives go on perfectly.

The primary role of the kidneys is to filter blood. The filtrate comes out as urine. The kidneys also play a role in ensuring that the bodys electrolytes are stable. This by extension results in stronger bones.

Diseases and lifestyle approaches may affect the proper functioning of the kidneys. This may result in kidney failure, infections, or stones. Baking soda can, therefore, be used to alleviate possible damage to the kidneys.

It should be noted here that the pancreas is known to produce some sodium carbonate that helps the kidney perform its function. In some cases, the kidney may itself produce sodium carbonate.

When this is no longer possible, external sources may be recommended.

Now, what is the recommended baking soda dosage for kidney diseases? Well, in summary, you need to take 1/2 teaspoon of baking soda daily for 4 days, and then followed by 7 more days of 1/4 teaspoon of baking soda a day.

David Wolfe outlines that this should be done in 3 phases. This can be as outlined below.

Who Needs Sodium Bicarbonate

If sodium bicarbonate is considered important for a persons treatment, then their physician will prescribe a dose of sodium bicarbonate in a tablet form. It can help be a buffer to decrease acid level in the blood. Be aware this medication is high in sodium and should not be used without physician approval. Problems with metabolic acidosis are more likely present in late stages of chronic kidney disease and in dialysis patients.

The Disadvantages Of Sodium Ascorbate

Sodium ascorbate is an alternate form of vitamin C, and may be used as a vitamin supplement. Sodium ascorbate is a more bioavailable form of the vitamin your body can absorb, access and use sodium ascorbate with greater efficiency than regular vitamin C. While taking sodium ascorbate may prove beneficial for people suffering from vitamin C deficiency, there are also a number of risks associated with taking sodium ascorbate supplements.

Alkali Supplementation In Ckd

The effects of alkali supplementation on the progression of renal disease have been tested only in small studies, with less than optimal design., A single-center randomized controlled but unblinded study in patients with late stage CKD and established acidosis demonstrated that correction of metabolic acidosis with alkali supplements slowed progression of CKD to ESRD. Two other small studies support the above findings., These and the prior animal studies raise the strong possibility that such treatment could retard progression. The sodium load given at the time of alkali supplementation has not been associated with an increase in BP, edema, or episodes of heart failure, likely owing to the fact that sodium excreted after sodium bicarbonate supplementation is greater compared with sodium chloride administration. The results of ongoing alkali supplementation trials are eagerly expected .

Ongoing clinical trials of sodium bicarbonate supplementation in CKD

A couple of studies using dietary interventions with fruits and vegetables, which lower the net dietary acid load, showed a similar role in improving markers of kidney injury expressed as urinary albumin, N-acetyl--d-glucosaminidase, TGF-, and endothelin 1., GFR was preserved in patients with hypertension-associated kidney disease who reduced their intake of dietary acid by consuming base-inducing fruits and vegetables or bicarbonate.
