Dietary Advice For Kidney Patients

Dietary treatment is an important aspect of care for all patients with kidney disease. It is necessary to meet with your dietician to discuss individual needs for your renal diet.

The following information will give you an overview of a renal diet, prior to dialysis, and also what to expect when you commence dialysis. You will be informed by the dietician when it is necessary to commence this diet. All patients have different dietary needs, so it is important to start this diet only under the guidance of the dietician.

A healthy balanced diet contains correct amounts of protein, carbohydrate, fat, vitamins and minerals. It is important that your diet is balanced and varied to keep you in optimum health.

Some of the main functions of the kidney that relate to the diet include: Excretion of waste products Control of fluid volume in the body Control of blood pressure.

When food and drinks are consumed, our bodies use what is needed and the rest is turned into waste products which can be excreted as urine. When your kidneys are not working properly, these waste products can build-up in your blood and cause complications, which will be discussed in the following sections.

Foods to avoid.

SALT

PROTEIN

PHOSPHATE

As with potassium, an elevated phosphate level will require you to reduce the intake of phosphate from your diet. It may also be necessary to take phosphate binding substances with your food to reduce the absorption of phosphate from the gut.

POTASSIUM

FLUIDS

Do Diabetics Have Kidney Problems

About 25% of people with diabetes eventually develop kidney disease. Diabetic nephropathy affects your kidneys ability to do their usual work of removing waste products and extra fluid from your body.

What is the most important symptom of early kidney disease from diabetes?

The earliest sign of diabetic kidney disease is an increased excretion of albumin in the urine. This is present long before the usual tests done in your doctors office show evidence of kidney disease, so it is important for you to have this test on a yearly basis. Weight gain and ankle swelling may occur.

Why do kidney patients need sodium bicarbonate?

Healthy kidneys help keep your bicarbonate levels in balance. Low bicarbonate levels can also cause your kidney disease to get worse. A small group of studies have shown that treatment with sodium bicarbonate or sodium citrate pills can help keep kidney disease from getting worse.

Causes Of Excessive Or Frequent Urination

Some of the causes of increased urine volume differ from those of too-frequent urination. However, because many people who produce excessive amounts of urine also need to urinate frequently, these two symptoms are often considered together.

The most common causes of urinary frequency are

The most common causes of polyuria in both adults and children are

What The Doctor Does

Doctors first ask questions about the person’s symptoms and medical history and then do a physical examination. What they find during the history and physical examination often suggests a cause of excessive urination and the tests that may need to be done .

Doctors ask about

  • Amounts of fluid drunk and urinated to determine whether the problem is related to urinary frequency or to polyuria

  • How long symptoms have been present

  • Whether any other urination problems are present

  • Whether the person is taking diuretics , including beverages that contain caffeine

Some obvious findings may give clues to the cause of frequent urination. Pain or burning during urination, fever, and back or side pain may indicate an infection. In a person who drinks large amounts of beverages that contain caffeine or who has just started treatment with a diuretic, the diuretic substance is a likely cause. A man who has other problems with urination, such as difficulty starting urination, a weak urine stream, and dribbling at the end of urination, may have a prostate disorder.

Creatine is a natural compound made in your liver. Its transported to your muscles where its used for energy. When creatine is used as energy, its degraded into creatinine, a metabolic byproduct.

In addition to its natural form, creatine is available as an oral supplement. Some athletes use these creatinine-producing supplements to help enhance athletic performance.

Anyone wishing to reduce their creatinine levels to improve their kidney function should not take creatine supplements. While creatine supplementation has been , there is a lack of long term studies on people with kidney disease.

Talk with your doctor before adding any supplement to your diet.

Acetaminophen Assays Interpretation And Methodology Acute Ingestion

The acute ingestion of acetaminophen in quantities of 150 mg/kg or greater may result in hepatic toxicity. However, the reported history of the quantity of a drug ingested as an overdose is often inaccurate and is not a reliable guide to therapy of the overdose. Therefore, plasma or serum acetaminophen concentrations, determined as early as possible, but no sooner than four hours following an acute overdose, are essential in assessing the potential risk of hepatotoxicity. If an assay for acetaminophen cannot be obtained, it is necessary to assume that the overdose is potentially toxic.

Interpretation of Acetaminophen Assays

  • When results of the plasma acetaminophen assay are available, refer to the nomogram in Figure 1 to determine if plasma concentration is in the potentially toxic range. Values above the line connecting 200 mcg/mL at 4 hours with 50 mcg/mL at 12 hours are associated with a probability of hepatic toxicity if an antidote is not administered.
  • If the predetoxification plasma level is above the line connecting 150 mcg/mL at 4 hours with 37.5 mcg/mL at 12 hours , continue with maintenance doses of Nefrosave. It is better to err on the safe side and thus this line, defining possible toxicity, is plotted 25% below the line defining probable toxicity.
  • If the predetoxification plasma level is below the line connecting 150 mcg/mL at 4 hours with 37.5 mcg/mL at 12 hours , there is minimal risk of hepatic toxicity, and Nefrosave treatment may be discontinued.
  • Mechanisms For The Effect Of Acetylcysteine On Renal Function After Exposure To Radiographic Contrast Material

    First Posted : November 14, 2007Last Update Posted : June 8, 2015

Millions of people receive radiographic contrast material for investigations like CT and coronary angiography. While considered safe in healthy patients, it can cause acute renal impairment. This is termed radiocontrast-induced nephropathy and is generally defined as an increase in serum creatinine over baseline of more than 25% or 0.5 mg/dL within 48 hrs. RCIN occurs in less than 2% of patients with normal renal function but is more common in patients with pre-existing renal damage.

The pathophysiology of RCIN is unclear. Possible mechanisms involve 1) reduced renal blood flow leading to acute tubular necrosis and 2) direct renal tubular injury by oxygen free radicals. Current prevention strategies focus on increasing renal blood flow and reducing oxidative stress. Patients at risk of RCIN currently receive fluids, a low dose of contrast, and variable and unproven doses of acetylcysteine.

    For Immediate Release:

Today, the U.S. Food and Drug Administration approved Farxiga oral tablets to reduce the risk of kidney function decline, kidney failure, cardiovascular death and hospitalization for heart failure in adults with chronic kidney disease who are at risk of disease progression.

Chronic kidney disease is an important public health issue, and there is a significant unmet need for therapies that slow disease progression and improve outcomes, said Aliza Thompson, M.D., M.S., deputy director of the Division of Cardiology and Nephrology in the FDAs Center for Drug Evaluation and Research. Todays approval of Farxiga for the treatment of chronic kidney disease is an important step forward in helping people living with kidney disease.

Chronic kidney disease occurs when the kidneys are damaged and cannot filter blood normally. Due to this defective filtering, patients can have complications related to fluid, electrolytes , and waste build-up in the body. Chronic kidney disease sometimes can progress to kidney failure. Patients also are at high risk of cardiovascular disease, including heart disease and stroke.

Farxiga was not studied, nor is expected to be effective, in treating chronic kidney disease among patients with autosomal dominant or recessive polycystic kidney disease or among patients who require or have recently used immunosuppressive therapy to treat kidney disease.

The FDA granted the approval of Farxiga to AstraZeneca.
