Can Prednisone Cause Gastrointestinal Ulceration
Undesirable side effects of prednisone therapy include gastric or duodenal ulcers, osteoporosis, and fractures, mental changes, polyarteriitis nodosa, diabetes, and fulminant infections . Similar side effects are known to occur as complications of therapy with cortisone or pituitary adrenocorticotrophic hormone.
Function Of The Adrenals
A part of the endocrine system, the adrenals are located on top of the kidneys. One major task of these organs is secreting hormones that serve several purposes in the body.
One of the major hormones secreted by the adrenals is cortisol . This is the hormone your body needs when under stress. Anytime stress comes into your life, your body responds the same way regardless of the origin of the stress.
The hypothalamic-pituitary-adrenal axis is activated, setting in motion a cascade of hormonal and chemical changes. At the end of this cascade, the adrenals secrete cortisol to fight the effects of stress.
Ordinarily, once the stress is over, the adrenals return to a state of homeostasis. When stress continues, the burden of producing more and more cortisol can lead to adrenal fatigue, where the adrenals can no longer produce sufficient cortisol and the effects of stress grow.
Initially, adrenal fatigue presents vague symptoms. As the condition worsens, so do the symptoms, until finally, they may become so severe as to render the person bedridden and/or unable to function.

The Need To Monitor Kidney Function With Certain Drugs
Experts have suggested that after the initial assessment of kidney function, physicians should consider regular monitoring after starting or increasing the dosage of drugs associated with nephrotoxicity, especially those used chronically in patients with multiple risk factors for impaired kidney function, Dr. Naughton noted. If there is any sign of kidney harm, the provider should review the medications you are taking in order to identify which one is causing the problem.
If multiple medications are present and the patient is clinically stable, physicians should start by discontinuing the drug most recently added to the patients medication regimen. Once that has been taken care of, further harm to the kidneys may be minimized by keeping blood pressure stable, staying hydrated, and temporarily avoiding the use of other medications that may cause nephrotoxicity.

If you are already taking blood pressure medications when you began taking prednisone, your healthcare provider may adjust these medications. They may just recommend monitoring your blood pressure to assess how your body is responding to prednisone.
If you develop high blood pressure while taking prednisone, your healthcare provider may recommend starting blood pressure medication to help.
Lifestyle changes can be implemented to help prevent high blood pressure. Eating a balanced diet of fruits, vegetables, whole grains, and healthy fats can help with maintaining a health weight.
It also helps to limit the total amount of salt in your diet. It is recommended to limit your salt intake 2 grams or 2000 milligrams sodium per day. This includes the sodium that naturally occurs in food, salt added during processing, and any salt you add to your foods.
Try these tips to help reduce your overall salt intake:
- Keep saltshakers off of the table and avoid adding salt after you’re done cooking.
- Cook with other herbs and spices that don’t have salt added to them, such as black pepper, oregano, cumin, onion powder, garlic powder, and any spice blend without sodium.
- When purchasing canned or processed foods, choose those labeled low sodium or no salt added.
- Read the label to see how many milligrams of sodium it has per serving and the serving size.
- Rinse can foods to help remove a lot of the extra salt that is added during processes to help preserve the food.

Study examines renal effects of steroids in bodybuilders
NEW YORK Anabolic steroids may help athletes gain muscle mass and strength, but this bulking up comes at the risk of serious kidney damage, according to research published in the Journal of the American Society of Nephrology. The findings indicate that the habitual use of anabolic steroids has potential harmful effects on the kidneys that were not previously recognized.
Anabolic steroid abuse is prevalent among both amateur and professional athletes. While these drugs are known to cause endocrine and liver dysfunction, until now their effects on the kidneys have not been appreciated, says Vivette DAgati, M.D., the Columbia University Medical Center researcher and practicing physician who led the study. Dr. DAgati and her colleagues recently conducted the study, the first of its kind to identify kidney disease following long-term abuse of anabolic steroids.
The investigators studied a group of 10 bodybuilders who took anabolic steroids for years. All developed protein leakage into the urine and severe reductions in kidney function. Kidney biopsies revealed that the bodybuilders had developed a condition called focal segmental glomerulosclerosis, a type of scarring within the kidneys. This disease typically occurs when the kidneys are overworked. The kidney damage in the bodybuilders has similarities to that seen in morbidly obese patients, but appears to be even more severe.

High Risk Of Infection With Steroid Treatment For People With Kidney Disease
Steroids caused a greater than expected increase in the risk of serious infections in the predominately young group of people who have immunoglobulin A nephropathy, an immune disease that leads to kidney failure in almost a third of patients.
In the largest trial conducted in this condition to date, people taking steroid tablets to treat IgA nephropathy have been found to incur a higher than expected risk of serious side effects, according to a study published in the Journal of the American Medical Association today.
The results showed that treatment with the oral steroid methylprednisolone caused an increased risk of infections as well as gastrointestinal and bone disorders. At the same time, the results of the study also suggest that the treatment may have a protective effect on kidney function.
While corticosteroids appear likely to have benefits on kidney function for people with IgA nephropathy, they also have important and potentially life-threatening risks. People should carefully weigh these up when deciding whether to use this treatment, said Professor Vlado Perkovic, Executive Director, The George Institute Australia and co-principal investigator of the trial.
IgA nephropathy is a disease where the kidney is damaged by the immune system when the antibody immunoglobulin A lodges in the kidneys. Up to 30 percent of all people with IgA nephropathy will eventually develop kidney failure requiring dialysis or kidney transplantation to prevent death.
